The SOAP does not tell me much about the intervention.

    3. Setting: So Helen is going to the therapist's home?????

    4. LTG #2 is the objective meal preparation???

STGs what is the occupation behind the STGs?

    1. LTGs are good but how do we measure, how do we know she completed them.

STG:The goals are good but, In order to make these STG work you have to define the setting, otherwise how do you know she is meeting her goals

A: In reality is not evident, because according to your assessment, she needs to improve her memory performance? whatever that means, and the objective results tell me that she has poor attention span and short term memory deficits. I need to know more about how this affects her daily habits, roles and routines.

    3. Agenda: Which screening? Shouldn't bullet #4 be the first step?

    4. The client is not just "meeting with the occupatinal therapist" in an outpatient clinic. Let's use professional language. For example: Maggie will be seeing in OccupationalTherapy as an Outpatient, 3x weeks for 1 hour sessions for 1 month.
