    1. Genotype-phenotype correlations of 573 VHL patients from 200 kindreds were analyzed. Higher risk of pheochromocytoma was associated with missense mutations that result in substitution of a surface amino acid (p<=0.004) than for patients with missense mutations that disrupt the structural integrity of the VHL gene product (pVHL) or other loss of function mutations (either germline deletions or truncating mutations). This missense mutation (c.461C>T) is within the protein core and was found in a single VHL family with 6 patients. One patient had retinal hemangioblastomas, 2 had cerebellar hemangioblastomas, 3 had renal cell carcinoma, 2 had pheochromocytoma. Only supporting evidence of pathogenicity because of cosegregation of disease with multiple affected family members in a gene definitively known to cause the disease (ACMG code: PP1).

    3. Genotype-phenotype correlations of 573 VHL patients from 200 kindreds were analyzed and confirmed that higher risk of pheochromocytoma is associated with missense mutations that result in substitution of a surface amino acid or that disrupt the structural integrity of the VHL gene product (pVHL). Age-related risks of retinal hemangoblastomas and renal cell carcinoma were higher in patients with nonsense or frameshift mutations. Southern blot and/or MLPA analysis by whole blood detected this mutation (c.562delC) was found in 2 family members with renal cell carcinomas, one with retinal angiomas (diagnosed at 25Y), and one with cerebellar hemangioblastoma (diagnosed at 24Y).

    1. The data of 18 patients with solid renal tumours associated with VHL disease were analyzed. All patients except for Patient 12 came from families with a definite VHL family history (the paper does not provide further details on this.) The patients came from 12 Chinese families and 5 were from the same family. Extrarenal manifestations of VHL disease were detected in 17 patients. Of 18 patients, 15 (including 6 asymptomatic) volunteered for DNA testing as previously described. Patients 1-5 had genetic testing and were found to have the same germline mutation c.223A>G (Asn78Ser). This missense mutation is noted as c.446A>G in the paper due to a difference in the VHL reference sequence notation used. It is unclear if these patients are related. Patients 4 and 5 had family study DNA test as their reason for treatment. Patient 1 was a 34-year-old male with a medical history of asynchronous bilateral solid renal tumors some which were further characterized as clear cell RCC, multiple renal cysts, CNS hemangioblastoma, pancreatic cyst, and epididymal cystadenomas. Patient 2 was a 44-year-old female with a medical history of asynchronous bilateral solid renal tumors some which were further characterized as clear cell RCC, multiple renal cysts, retinal angioma, and pancreatic cysts. Patient 3 was a 43-year-old male a medical history of synchronous bilateral solid renal tumors some which were further characterized as clear cell RCC, multiple renal cysts, retinal angioma, and pancreatic cysts. Patient 4 was a 52-year-old female a medical history of synchronous bilateral solid renal tumors, multiple renal cysts, retinal angioma, and pancreatic cysts. Patient 5 was a 46-year-old female with a medical history of clear cell RCC, multiple renal cysts, and pancreatic cysts.

    1. A previous study of 26 Japanese, VHL families was extended to 41 additional families. Germline mutations were detected in 55 of 77 Japanese families using SSCP, direct sequencing, and Southern Blot analysis of peripheral blood samples. The L118P (c.353T>C) germline missense mutation was found in a VHL type 1 family presenting with renal cell carcinoma (2/2) and pancreatic tumor or cyst (unspecified) (1/2) (family ID 83). This family was previously reported in PMID 8634692.

    1. One year later, a 10-year-old sister of the proband (II-2) was referred to our service because of palpitations, weight loss, diaphoresis, and shooting pain attacks that had started about two weeks earlier. Based on this clinical picture and because of her family history, a pheochromocytoma was suspected immediately. Her urinary metanephrines were elevated to 12.2 mg/g creatinine (< 1 mg/g). A CT of the abdomen revealed a mass in the left adrenal; there were no lesions in the pancreas or the kidney. This mass was also positive on MIBG scintigraphy. As in her brother, there was no suggestion that the paraganglia would also be involved. Her MRI of the brain, and her echocardiogram were both normal. She was then treated with an alpha-blocker (prazosin) in preparation for an open adrenalectomy. Histology and immunohistochemistry confirmed diagnosis of a pheochromocytoma in the left adrenal gland.

      PatientID: II-2

      KindredID: 1

      Case: F, 10Y0M, Ethnicity Unknown

      DiseaseAssertion: Adrenal pheochromoyctoma

FamilyInfo: The patient’s brother (II-1) was found to harbor the same germline mutation the patient harbored and had bilateral adrenal pheochromocytoma at 14Y. The same germline missense mutation was detected in another sister (II-4) with right adrenal pheochromocytoma and her asymptomatic daughter (III-1). The father was not available for genetic testing because he had died, apparently secondary to a hypertensive crisis. The five other family members which underwent genetic testing did not have the mutation.

    1. Peripheral blood samples were collected for mutation analysis of the RET proto-oncogene and VHL tumor suppressor gene. Genomic DNA was extracted from whole-blood samples using the QIAamp blood kit (Qiagen, Hilden, Germany) according to the manufacturer's instructions. Polymerase chain reaction (PCR) amplification of all exons and the flanking splice junctions of RET and VHL was performed, followed by direct bidirectional DNA sequencing. One hundred unrelated controls with a similar ethnic background were also included in this study

    1. A 20-year-old Caucasian woman presented to our institution with decreased vision in the right eye (RE) with a 4-month evolution. The patient’s vital signs were within normal limits, and no abnormalities were noticed upon physical and neurological examination. Similarly, her past medical history was unremarkable. Her best corrected visual acuity (BCVA) was 50 ETDRS (Early Treatment Diabetic Retinopathy Study) letters in the RE and 84 ETDRS letters in the left eye. Her intraocular pressure (IOP) was normal in both eyes, and the result of her anterior segment examination was unremarkable. Upon ophthalmoscopic examination, a red-colored globular lesion of 3 disk diameters (DDs) with prominent feeder vessels was noticed in the superior temporal region of the retina in the RE (Fig. 1). The fellow eye was normal on fundus examination, and no other lesions were found in the posterior pole or periphery. Optical coherence tomography (OCT) was carried out using the DRI OCT Triton™ SS-OCT device (Topcon Medical, Tokyo, Japan).

    5. In a study of 426 unrelated VHL patients, 111 were discovered to have alterations in the VHL gene. 18 novel variants were identified in VHL patients, but none were present in 200 unaffected control individuals. DNA samples from peripheral blood leukocytes underwent polymerase chain reaction (PCR) amplification, denaturing high-performance liquid chromatography (DHPLC), and direct sequence analysis of the entire coding sequence and intron-exon boundaries. The R161Q (c.482G>A) germline missense mutation of the VHL gene was found in a 8 year old VHL patient with pheochromocytoma (index case 244).

    1. Molecular and clinical analysis of 126 French VHL families tested for renal involvement revealed 92 different mutations. 90 of these families had renal involvement. The p.L158P (c.473T>C) germline variant of the VHL gene was detected in an individual (ID 5342, Family 252). They presented with renal cell carcinoma. The paper states that this individual is previously reported in PMID 12202531 and/or 9829912.

This paper presented a pheochromocytoma detected by autopsy. The index case was a 26-year-old male who presented to the emergency department complaining of obstipation, severe headache and abdominal pain. The family background of the parents and siblings history is unavailable since the patient was an orphan. An autopsy revealed bilateral pheochromocytoma and acute myocardial infarction. Molecular genetic analyses for germline mutations of the VHL gene were performed after DNA extraction from normal liver tissue using standard methods. A mutation in VHL gene R167W (c.499C>T) was detected.

    1. 41 non-related patients with retinal capillary hemangioma presenting in the Benjamin Franklin University Eye Clinic between 1988 to 1999 Germany were evaluated for the presence of other VHL lesions. VHL disease was diagnosed in 33 patients based on the presence of other severe VHL lesions (spinal hemangioblastoma, cerebellar hemangioblastoma, renal cysts, pancreatic cyst, renal carcinoma, and pheochromocytoma), molecular genetic testing, and/or family history. VHL germline mutations were identified in 27 out of 29 VHL families screened with molecular genetics. Blood samples were genetically tested using PCR-SSCP sequencing. One patient with VHL-related retinal angiomatosis was found to have germline frameshift deletion (c.210delG) mutation in the VHL gene. 1. In a study of 114 unrelated VHL families, 85 germline mutations were found. VHL mutations were detected in affected family members, but not in unaffected family members or 96 normal individuals. This missense mutation was found in a VHL type 1 family of 10 individuals (family ID 1239), using PCR and SSCP sequencing. The mutation of c.351G>T was referred to in the paper by c.564G>T due to a difference in transcript sequence. Seven of 10 individuals were identified with CNS hemangioblastomas and renal cell carcinoma. One identified with retinal hemangioblastomas. This is possibly previously published in PMID 9829911. Supporting evidence of pathogenicity because cosegregation with disease in multiple affected family members in a gene definitively known to cause the disease (ACMG code: PP1).

    2. In a study of 114 unrelated VHL families, 85 germline mutations were found. VHL mutations were detected in affected family members, but not in unaffected family members or 96 normal individuals. This frameshift mutation was found in 3 family members with VHL (Family ID 3575), using PCR and SSCP sequencing. The mutation of c.448delA was referred to in the paper by c.661delA due to a difference in transcript sequence. 2/3 members had retinal angiomas (hemangiomas), 1 had CNS hemangioblastoma and 1 had renal cell carcinoma.

    1. Von Hippel-Lindau (VHL) is a tumor suppressor that negatively regulates the production of angiogenic factors. Mutations in the VHL gene cause VHL syndrome, which is characterized by highly vascularized tumors. Here we report a c.464T>A mutation of the VHL gene in three patients with hemangioblastoma from a Chinese family. This mutation was not reported previously and was absent in the unaffected family members. The mutation is predicted to cause Val to Glu substitution at VHL protein residue 155 in a conserved region. Previous biochemical studies demonstrated that residue Val-155 was critical for VHL protein binding to chaperonin TRiC/CCT, an essential step for proper VHL protein folding. Our finding of naturally occurring VHL V155E mutation in patients with VHL syndrome supports the functional importance of Val-155 residue in VHL protein and illustrates the diversity of VHL gene defects underlying VHL syndrome.

    1. We have described the development and validation of a new gene-expression diagnostic test called Pheo-type for classification of PPGL tumors based on the underlying genetic driver of the disease. Our assay confirms that the predominant gene-expression subtypes of PPGL are closely linked to tumor genotype and for the first time show that classification can be achieved using a relatively small gene set. Pheo-type is similar to other clinical assays developed on the Nanostring platform for subtyping of solid tumors (50–52). As with these other studies, we have shown that robust gene-expression analysis can be achieved from small amounts of FFPE tissue material generating data that are comparable to other gene-expression platforms. The assay is rapid, largely automated, and cost effective, enabling the generation of results in 2 days from RNA extraction to data analysis, facilitating its implementation in a routine diagnostic laboratory.

    1. Here, we report imaging findings in a VHL patient and in 3 consecutive family members

Four patients from the same family carrying missense VHL mutation c.509 T>G were screened for VHL related lesions. Patient 1, 30Y F, had undergone treatment for pancreatic endocrine tumors and pheochromocytoma. Patient 2, 30Y M, was affected by retinal angioma, bilateral adrenal pheochromocytoma and sacral, intercarotid paraganglioma. Patient 3 underwent treatment for pheochromocytoma. Investigations revealed additional pancreatic endocrine tumor and a pheochromocytoma. Patient 4 had an endocrine pancreatic tumor and a pheochromocytoma. ACMG: 'PP4' because the patients' phenotypes and family history are highly specific for a disease with single genetic etiology.

    1. This study reports 1,548 germline and somatic mutations from 945 VHL families, including 30 previously unpublished kindreds from The Netherlands (six novel mutations). This mutation (p.P86S (c.256C>T)) was detected by direct sequencing and MLPA in 2 VHL kindreds. One family had pheochromocytoma, renal cell carcinoma, and retinal angiomas (case no. 182). The other family had hemangioblastomas of the central nervous system (case no. 177). ACMG evidence codes: 'PP2' because they observe a missense variant in a gene where missense variants are a common mechanism of disease, 'PP4' because the patient's phenotypes or family history are highly specific for a disease with single genetic etiology.

    1. We report two young male twins with simultaneously manifesting haemangioblastomas of the right cerebellar hemisphere. In one, the neoplasm was associated with medullary and spinal cord solid tumours; in the other, with a nasopharyngeal bleeding fibroangioma, an association not described previously.

      DiseaseAssertion: VHL

FamilyInfo:  2 young male twins were examined with the VHL gene mutation with simultaneously manifesting hemangioblastomas of the right cerebellar hemisphere. Both parents were examined with no VHL mutation, detecting a de novo mutation in the twins. A third sibling, two years older, presented with a cafè au lait skin spot. Grand mother had bilateral high myopia and glaucoma. The medical history of the pedigree shows that member (I-1) had severe hypertension and died at 60 from a stroke, member (I-2) died at 40 from ovarian carcinoma, and member (I-3) died at 54 from clinically diagnosed intracerebral haemorrhage.

Another small mural nodule was present in the infero-posterolateral portion of the right cerebellar hemisphere. CT disclosed a round homogeneous, hyperintense nodule at T1 in the spinal cord. Raised intracranial pressure due to the compression of aqueduct from the tumour accounted for hydrocephalus and enlarged lateral and 3rd ventricles.

    2. Thirty-five unrelated patients suspected of having von Hippel-Lindau disease were analyzed and 32 VHL gene variants were found in 35 patients using PCR and SSCP by peripheral blood leukocytes. An association of clear cell renal carcinoma development with a relatively high loss of structural stability in VHL protein missense mutations was discovered. This nonsense mutation was found in a patient with retinal hemangioblastoma, cerebellar hemangioblastoma, and pancreatic cysts (patient no. F3). Patient has family history of paragangliomas, epididymial cysts, and clear cell renal carcinoma. Paper gives variant by protein sequence: L135*. NM_000551.3:c.404T>A and NM_000551.3:c.404T>G both could result in this protein change. Unclear which is correct to the given patient.

    1. missense

A cohort of 314 patients with pheochromocytoma and functional paraganglioma had blood samples collected and all five major pheo-pgl susceptibility genes (RET, VHL, SDHB, SDHD, and SDHC) were screened for mutations. 25 patients had germline VHL mutations, 16 of which had a family history and 9 patients had an apparently sporadic presentation, 11 were male and 14 were female, the median age of diagnosis is 23.9 and range of diagnosis was 4-49. Out of the 25 patients 23 had adrenal tumours, 4 had para-adrenal tumours, and 9 had other VHL lesions (renal tumours and hemangioblastomas). 1 patient with positive family history and/or a syndromic presentation was found with pheochromocytoma or functional paraganglioma and a germline missense VHL mutation (p.L184H (c.551T>A), referred to in the paper by c.608T>A due to a difference in transcript sequence). Other phenotypes unique to the patient are unknown. ACMG: PP4

    1. Genomic DNA was purified from peripheral blood leukocytes using a QIAamp DNA Blood Mini Kit (Qiagen GmbH, Hilden, Germany) according to the manufacturer’s instructions. The three exons of the VHL gene as well as their flanking introns were amplified using the following primers: 1F 5′-IndexTermCGCGAAGACTACGGAGGT-3′ and 1R 5′-IndexTermCTTCAGACCGTGCTATCGTC-3′; 2F 5′-IndexTermCTCCCAAAGTGCTGGGATTA-3′ and 2R 5′-IndexTermTGGGCTTAATTTTTCAAGTGG-3′; 3F 5′-IndexTermAGTTGTTGGCAAAGCCTCTT-3′ and 3R 5′-IndexTermAAGGAAGGAACCAGTCCTGT-3′. We confirmed mutations in both strands by direct sequencing with the primers that were also used in the PCR. PCR was carried out using a thermal cycler (Model 9700; Applied Biosystems, Foster City, CA, USA) as follows: 35 cycles of denaturation at 94 °C for 30 s, annealing at 60 °C for 30 s, and extension at 72 °C for 1 min. The amplicon was purified using Agencourt AMPure XP (Beckman Coulter Genomics, Danvers, MA, USA) and direct sequencing was performed using the BigDye Terminator Cycle Sequencing Ready Reaction kit on an ABI Prism 3130 genetic analyzer (Applied Biosystems). All variants were confirmed based on the National Center for Biotechnology Information single-nucleotide polymorphism database and the Human Gene Mutation Database. The method for detection of VHL germline mutations, including that for multiple ligation-dependent probe amplification, used in Samsung Hospital was previously reported.11

    2. VHL mutational analysis Genomic DNA was extracted from peripheral blood leukocytes using the semi-automated Quickgene system (Fujifilm, Tokyo, Japan). The entire VHL coding sequence (three coding exons) as well as the intron/exon junctions were amplified by PCR using pairs of primers derived from the VHL published sequence. Primer sequences and annealing temperatures are available on request. Each 25 µl PCR amplification reaction contained 1X buffer, 200 ng of genomic DNA, 0.2 mM of each dNTP, 2 U Taq polymerase, 1 mM of forward and reverse primers, and 1.5 mM MgCl2. PCR products were analysed in 1.5% agarose gels from which the bands with the amplified templates were excised, and the DNA subsequently purified with the help of the Qiaex II kit (Qiagen, Hilden, Germany). Direct automated sequencing of VHL was performed with the BigDye Terminador Cycle Sequencing kit (Applied Biosystems, Foster City, CA, USA), adding about 15 ng of template DNA in each reaction and using a temperature program that included 25 cycles of denaturation at 95°C for 30 s, annealing at 50°C for 15 s, and extension at 60°C for 4 min. All samples were analized in an ABI Prism 310 Genetic Analyzer (Applied Biosystems). Wild-type (Ensembl Transcript ID: ENST00000256474) and mutated VHL sequences were compared manually.

    1. a deleting mutation in the first exon (nt. 394-406) of the VHL locus, truncating 16 amino acids (N61-77) from the first beta sheet in the VHL protein.

The patient presented at age 43 with a new left varicocele having already suffered one episode of gross hematuria. An abdominal ultrasonography revealed the presence of a left renal mass and a 2-cm renal mass in his right kidney. CT scan demonstrated two small 4-mm lung lesions. This was followed by a left radical nephrectomy. Pathological examination of the mass demonstrated a clear-cell RCC. A 1-cm isolated metastatic nodule was also found in the adrenal gland. Two years later, the lung lesions had increased to 5 mm, a further three 2-mm nodular lesions had appeared in the lungs and no growth in the small right renal lesion was found. Following this, MR imaging demonstrated an intradural intramedullary lesion (later confirmed to be consistent with a hemangioblastoma from the inferior portion of the T-5 to T-6) with a prominent feeding vessel. The patient underwent a T5–8 laminectomy. Clinical diagnosis of VHL was reached at this point. Genetic analysis found a deleting mutation in the first exon (nt. 394-406) of the VHL locus, truncating 16 amino acids (N61-77) from the first beta sheet in the VHL protein. This mutation was translated to the updated reference sequence (NM_000551.3) as p.P61_C77del (c.181_231del). This mutation was not present in any of the family members tested including the patient’s two children. At this point, MRI showed no masses, ophthalmological examination showed normal results and a congenital malformation with C-1 occipitalization and C2–3 fusion were identified. The patient underwent resection of the right renal mass, which was consistent with a clear-cell carcinoma. One year later, two cerebral lesions were diagnosed and were consistent with RCC metastases in the right occipital lobe for which he received radiation therapy. No new lesions had appeared in the spinal cord and his chest disease was found to be stable. ACMG evidence codes: 'PM6' since this mutation was declared to be non-familial but kindred testing was not specified (assumed de novo), 'PP4' since the patients phenotypes are highly specific for a disease with a single genetic etiology.

    1. Patient 2 A 17-year-old girl with a history of primitive headache, intense nocturnal sweating and 2-month-long amenorrhoea and elevated blood pressure presented a 7-cm-diameter right adrenal mass at abdominal sonography, confirmed by MRI. The mass showed inhomogeneous density: hypodensity in T1 and hyperdensity in T2 and after Gadolinium contrast. Immunometric assays indicated very high levels of chromogranin A (242 UI/L; NR 2–18) and catecholamines: noradrenaline levels were 14·460 nmol/day (NR: 90–470), and adrenaline and dopamine levels were in the normal range. A 123I-MIBG scintigraphy showed a high tracer uptake in right adrenal gland, as expected. The patient underwent surgical removal of the mass, which was 8 × 6 × 5 cm in size. Immunohistochemical assays indicated that the tumour was positive for chromogranin A and synaptophysin, compatible with a diagnosis of PHEO. Follow-up blood test after 2 years was within normal limits, except for a still moderate elevated CgA (67 UI/L, NR 2–18). The family history was negative for PHEO or other neuroendocrine tumours.

    1. 33 patients diagnosed with VHL disease, according to (Maher et al. 1990), were tested for germline VHL mutations. Genetic testing was performed on high-molecular-weight DNA from peripheral blood using Southern analysis and PCR. One patient was found to harbour the 508G>T (Val170Phe) VHL gene mutation, documented in the paper as 721G>T due to a difference in transcript sequence. This patient presented with a cerebellar hemangioblastoma. As well, a somatic mutation presenting in their tumor as p.N193M*9 (c.576del13). ACMG evidence codes: 'PP4' since the patient presents phenotypes highly specific for a disease with a single genetic etiology.

    1. Poorer survival has been associated with loss of function events (frameshift or nonsense mutations) compared with wild-type, missense mutations, and mutations of unknown biological consequence (25, 32) although numbers of cases in the LOF group were very small. We found no prognostic effect of VHL mutation but a trend (although not quite statistically significant) was seen for increased frequency of microvascular invasion in tumors where the presumed deletion would occur essentially from the elongin binding region onwards, possibly indicating a role for either HIF-regulated factors or other VHL substrates in this aspect of tumorigenesis. The largest previous study also found no relationship between clinicopathologic factors and VHL mutation/LOH in cRCC (26, 35). In marked contrast, however, is that 0 of 11 cRCC cases with methylation had an intragenic mutation whereas we found 14 of 19 methylated cases to have a confirmed mutation, although their mutation and methylation frequencies overall were much lower (51% and 5.4%, respectively). However, a significantly better cancer-free and overall survival was seen in patients with stage I to III cancer with VHL alteration (mutation or methylation) but wasn't seen in stage IV disease (35). Our data fails to confirm this although when stage IV patients were included a similar trend was seen. Differences between studies might reflect samples sizes, length of follow-up, and postoperative treatment.
    1. The VHL mutation pattern includes point mutations, small deletions and large deletions.

This paper analyzed the VHL gene of 21 patients who had clinical a clinical diagnosis of VHL, familial pheochromocytoma, or sporadic pheochromocytoma. 7 patients were from 3 families, while the remaining 14 were unrelated. Mutations were detected by realtime quantitative PCR (RQ-PCR) assays using peripheral venous blood. The mutation (c.533T>A) was found in 1 individual (female, 21Y), without family history, who presented with pheochromocytoma and hypertension. This case was also later presented in PMID: 24735130 (Pai et al, 2014). ACMG evidence codes: 'PP2' because they observe a missense variant in a gene where missense variants are a common mechanism of disease.

    1. 56 male patients age 12 to 76 years, from 40 families with VHL were examined as part of a larger screening program for VHL. VHL mutations were identified in 16 families. Individual patient phenotype-genotype data was not available, but the mutations found and whether the corresponding patient had epididymal cystadenoma (EC) was reported. A p.Asn78His (c.232A>C) VHL mutation was found in a VHL patient with EC. This mutation was referred to in the paper as c.445A>C due to a difference in transcript sequence.

    1. One hundred and sixteen unrelated VHL disease patients have been investigated by molecular genetic analysis. Twenty-two of these were previously shown to have germline deletions on Southern analysis, and in the present study the remaining 94 patients were screened for intragenic mutations via SSCP and heteroduplex analysis of 855 bp of the VHL coding sequence and 800 bp of the 3' UTR. An intragenic mutation was identified in 55 (59%) out of 94 patients without germline deletions on Southern analysis. In all patients with multiple affected family members the missense mutation segregated with the disease. Large deletions or intragenic mutations predicted to cause a truncated protein were found in 36 of 53 families without pheochromocytoma (VHL type 1) and 2 of 12 families with pheochromocytoma (VHL type 2). This study also investigated the frequency of different mutation classes between kindreds with and without pheochromocytoma. The p.R79P (c.236G>C) variant (noted as p.R150P (c.449G>C) in the paper due to a difference in reference transcript used) was found in at least one patient in kindred 150. No familial phenotypic information could be inferred from the paper.

    1. Whole blood was collected for genomic DNA analysis and VHL gene sequencing, which confirmed VHLs (Fig. 2). The VHL gene nucleotide sequence was obtained by EMBL Nucleotide Sequence Database [BBC+00] (http://www.ebi.ac.uk/embl/index.html; ENST00000256196, sequence: {"type":"entrez-nucleotide","attrs":{"text":"JQ821733","term_id":"392055954","term_text":"JQ821733"}}JQ821733). Sequencing was performed by polymerase chain reaction using the following primers: F: TACTACAGAGGCATGAACACC and R: CCCCTAAACATCACAATGC. In silico analyses by the mutation analyzer 2.0.28 (https://mutalyzer.nl) suggested the pathogenic potential of this variant. Pheochromocytoma was excluded by 24-h metanephrine and serum catecholamine measurement.

ArticleReferenceSequence: Not specified – Assumption: variants assumed to have been documented by the author under reference sequence NM_000551.3

    1. Case presentationFirst onsetA 19-years-old female was first found with occupation about 6.2 cm × 5.7 cm in the right adrenal five years ago based on CT scan at a regular physical examination, accompanied with fever and headache with high blood pressure. Tumor resection was performed, and pathological diagnosis was right pheochromocytoma. The patient was regularly subjected to follow-up.Secondary onsetDuring this hospitalization, CT scan found multiple tumor occupation in left adrenal and paraganglion region zones with round nodular shadows. Meanwhile, she had fever and headache with high blood pressure. Based on medical history and examination, she was diagnosed as left pheochromocytoma. Meanwhile, the occupying lesions in right lung and pancreas were found, located in extrabasal segment of inferior lobar and head respectively.Case characteristicsThe case characteristics were summarized: 1. A 12y female; 2. Tumor occupation was first found in right adrenal; 3. Tumor resection was performed and the pathological diagnosis was pheochromocytoma; 4. Multiple round nodular shadows were found in left adrenal and paraganglion region zones after five years; 6. The symptoms were fever and headache with high blood pressure; 7. The diagnosis was pheochromocytoma (PCC) and paraganglioma (PGL) (Combined PPGLs); 8. The occupying lesions located in right lung and pancreas were scanned and diagnosed with masses.Diagnostic methodThe patient was followed up regularly. Based on personal medical history, symptoms and imaging examinations, the patient was diagnosed as PPGLs combined with right lung and pancreas occupation.Multidisciplinary team discussion based on professional board indicated the following views: (1) The diagnosis was possibly considered as atypical VHL type II. Further qualitative diagnosis relied on genetic examination; (2) Other lesions like retina tumor should be examined. (3) PPGLs was surgically resected for pursuing pathological diagnosis. (4) The occupying lesions in right lung and pancreas were performed with puncture biopsy for further diagnosis. The diagnosis and treatment workflow were shown in Fig. 1.

    1. Materials and MethodsPatients and Sample CollectionOur study included 314 PPGL patients from the Peking Union Medical College Hospital, Beijing, China. Most patients had undergone surgery and were histopathologically diagnosed with PPGL. For the patients with unresectable or metastatic tumor, PPGL diagnosis was established on the basis of clinical features (headache, palpitations and sweating), increased 24-h urinary catecholamine excretion or plasma metanephrines and imaging tests. 99mTc-hydrazinonicotinyl-Tyr3-0ctreotide (99mTc-HYNIC-TOC) scintigraphy, Iodine-131 metaiodobenzylguanidine scintigraphy and contrast-enhanced thoracoabdominal-pelvic computed tomography (CT) were performed for almost all PPGLs. Some patients underwent head and neck enhanced CT or head and neck enhanced MRI examination. 18F-FDG-PET/CT or 68Ga-DOATATATE-PET/CT were performed to patients suspected of metastasis or PPGLs with multiple lesions. Blood samples of the patients were collected, with informed consents obtained from the patients and approval from the medical ethics committee of the hospital.Genomic DNA Preparation and Sanger SequencingGenomic DNA was extracted from the peripheral blood of PPGL patients by using blood DNA Midi Kit (Omega Bio-Tek, Norcross, Georgia, USA). For patients with metastatic PPGL, all the coding sequences and the intro-exon junctions of SDHB ({"type":"entrez-nucleotide","attrs":{"text":"NM_003000.3","term_id":"1732746310","term_text":"NM_003000.3"}}NM_003000.3) were amplified and sequenced on an ABI3730 DNA analyzer (Applied Biosystems, CA, USA). For patients with head or neck PPGL, all the coding sequences and the intron-exon junctions of SDHB and SDHD ({"type":"entrez-nucleotide","attrs":{"text":"NM_003002.4","term_id":"1519245278","term_text":"NM_003002.4"}}NM_003002.4) were amplified and sequenced. As for patients with clinically-diagnosed hereditary syndromes, such as multiple endocrine neoplasia Type 2 (MEN2) and Von Hippel-Lindau (VHL) disease, hot spots (10, 11, 16 exons) of RET ({"type":"entrez-nucleotide","attrs":{"text":"NM_020630.4","term_id":"126273513","term_text":"NM_020630.4"}}NM_020630.4) and all exons, including intro-exon junctions of VHL ({"type":"entrez-nucleotide","attrs":{"text":"NM_000551.3","term_id":"319655736","term_text":"NM_000551.3"}}NM_000551.3), were amplified and sequenced, respectively. The primers used for the PCR amplification were listed in Supplementary Table 1.Next-Generation SequencingFor all the patients except those whose variants were detected by the aforementioned Sanger sequencing, next-generation sequencing covering SDHA ({"type":"entrez-nucleotide","attrs":{"text":"NM_004168.4","term_id":"1519245104","term_text":"NM_004168.4"}}NM_004168.4), SDHB ({"type":"entrez-nucleotide","attrs":{"text":"NM_003000.3","term_id":"1732746310","term_text":"NM_003000.3"}}NM_003000.3), SDHC ({"type":"entrez-nucleotide","attrs":{"text":"NM_003001.5","term_id":"1779948875","term_text":"NM_003001.5"}}NM_003001.5), SDHD ({"type":"entrez-nucleotide","attrs":{"text":"NM_003002.4","term_id":"1519245278","term_text":"NM_003002.4"}}NM_003002.4), SDHAF2 ({"type":"entrez-nucleotide","attrs":{"text":"NM_017841.4","term_id":"1779948864","term_text":"NM_017841.4"}}NM_017841.4), VHL ({"type":"entrez-nucleotide","attrs":{"text":"NM_000551.3","term_id":"319655736","term_text":"NM_000551.3"}}NM_000551.3), RET ({"type":"entrez-nucleotide","attrs":{"text":"NM_020630.4","term_id":"126273513","term_text":"NM_020630.4"}}NM_020630.4), MAX ({"type":"entrez-nucleotide","attrs":{"text":"NM_002382.5","term_id":"1653960896","term_text":"NM_002382.5"}}NM_002382.5), TEMEM127 ({"type":"entrez-nucleotide","attrs":{"text":"NM_017849.4","term_id":"1777425445","term_text":"NM_017849.4"}}NM_017849.4), FH ({"type":"entrez-nucleotide","attrs":{"text":"NM_000143.4","term_id":"1779521770","term_text":"NM_000143.4"}}NM_000143.4), NF1 ({"type":"entrez-nucleotide","attrs":{"text":"NM_001042492.3","term_id":"1732746177","term_text":"NM_001042492.3"}}NM_001042492.3), and KIF1B ({"type":"entrez-nucleotide","attrs":{"text":"NM_015074.3","term_id":"189339259","term_text":"NM_015074.3"}}NM_015074.3) was performed to examine the potential pathogenic germline variations. The probes of target regions for relevant genes were designed against Agilent available online (http://www.agilent.com). Agilent SureSelectXT custom kit (Agilent Technologies, Palo Alto, CA) was used to generate sequence library according to instructions. Briefly, fragments of 180-280bp were produced by using a hydrodynamic shearing system (Covaris, Massachusetts, USA). The DNA fragments were end-repaired, A-tailed and adapter-ligated for Illumina sequencing. Then, size selection, PCR amplification and library hybridization were performed. Each captured library with an index was loaded onto the Illumina HiSeq X platform (Illumina Inc., San Diego, CA), and 150-bp paired-end reads were generated. The fastq files were subjected to quality control to exclude unwanted sequences, including adapter-contaminated or low quality or unrecognizable nucleotides. The fastq files were aligned against the Human Reference Genome (UCSC hg19) by using the Burrows-Wheeler Aligner (BWA). Then, the SAM tools (11) were used to generate the final BAM file (12) to calculate the sequence coverage and depth by sorting the BAM files and performing repeated marking, partial realignment and base quality recalibration. At last, the Variant Call Format files obtained in the previous step were annotated by ANNOVAR (13).Annotated variants with coverage over 10×, mutant allele frequency (MAF) < 0.01 in the 1,000 Genomes databases and in the exonic or splicing region (10 bp upstream and downstream of splicing sites) were retained (14). The synonymous variants were excluded. Non-synonymous SNVs were retained if at least two of the functional predictions by PolyPhen-2, SIFT, MutationTaster and CADD showed the SNV was damaging. “Benign” or “likely benign” variants in ClinVar were eliminated (15–18). American College of Genomic Medicine (ACMG) guidelines were used for the classification of variant pathogenicity (19). All the variants were confirmed by Sanger sequencing.

    1. Case 12In 1993, a 39-yr-old female with recent onset of hypertension was diagnosed with a right pheochromocytoma. At age 36, a suspected left pheochromocytoma was noted by CT and increased urinary metanephrines was confirmed at age 39 by [123I]MIBG scintigraphy. This imaging showed in addition an uptake located in the neck. MRI confirmed a left adrenal tumor and a left 2-cm vagal paraganglioma that was removed and confirmed histopathologically. Genetic testing identified the VHL p.Tyr156Cys mutation. No other tumor of the VHL spectrum was discovered. Three of her four sons were found to carry the mutation. Clinical screening of all 3 disclosed a 2.4-cm abdominal paraganglioma in one of them but no lesion in the other brothers.

This study examined the expression of hypoxia related genes in renal cell carcinoma. The authors analyzed the tumor and surrounding healthy tissue in 10 patients who had radical nephrectomy due to RCC. DNA was isolated from blood, tumor and surrounding healthy tissue for sequencing VHL gene. Only 1 patient had a hereditary form of RCC with a substitution of leucine to proline at position 25 (P25L) as a polymorphism of the VHL gene. ACMG: 'PP4' because the patient' phenotype is highly specific for a disease with single genetic etiology.

    1. Intron1 IVS1-38 C>T

27 index patients suspected of having VHL disease from unrelated Chinese families and their relatives were studied. The nucleotide were numbered according to the sequence numbering by Latif et al, 1993 and this was converted to NM_000551.3. The paper did not differentiate between a pancreatic cysts and tumor and presented these together as a 'pancreatic lesion'. The paper summarized the phenotypes of the families together. Family 20 was found with an intron mutation in the VHL gene (paper reported as IVS1-38C>T which was converted to recommended HGVS notation): both members had CNS hemangioblastomas and retinal capillary hemangioblastomas.

This intron variant (IVS1-38 C>T) is unknown. The entered coordinates do not agree with the intron/exon boundaries for the selected transcript; The base at c.340 not "C". Authors were contacted and could not confirm.

    1. 32 c.341-2A>C IVS2-2A>C Familial VHL

Between 1994 and 2001, the Dutch VHL working group conducted DNA analysis on 146 probands with VHL-associated manifestations. DNA of probands was extracted from peripheral blood samples. Exons 1, 2 and 3 of the VHL gene and their flanking untranslated and intronic coding sequences were amplified by PCR and subjected to semi-automated sequence analysis. Screening for structural rearrangements, including deletions, involving the VHL gene was performed by Southern blot analysis and the multiplex ligation-dependent probe amplification (MLPA) assay. Deletions encompassing the entire VHL gene were confirmed by FISH analysis on lymphocytes from affected individuals. VHL mutations were identified in 43 families. This splice mutation c.341-2A>C was found in a VHL patient with hemangioblastomas of the retina and central nervous system, renal cysts and RCC, and pancreatic cysts (family no. 32). They were described as being familial VHL however no other members were described. ACMG evidence codes: 'PSV1' because they observe a splice-site variant in a gene where loss-of-function is known mechanism of disease, 'PP4' because the patients' phenotypes or family history are highly specific for a disease with single genetic etiology.

    1. Case 2This 10 year old girl presented with intermittent fever for the past one month. Her birth history was unremarkable. She was the fourth child of nonconsanguineous parents. Her family history was not significant for tumor occurrence. Physical examination revealed a blood pressure level of 160/100 mmHg (95p: 120/79 mmHg) and a heart rate of 110 beats per minute. Laboratory tests showed an elevated 24 h urinary VMA level of 83 mg/day. Abdominal MRI revealed a 44x33 mm, well circumscribed mass with a necrotic core in the left adrenal gland. Subtotal adrenalectomy was performed and histologic examination showed that the tumor was PCC. During two years of follow-up, a 20x19x12 mm mass was detected in the right adrenal gland on abdominal MRI. PET-computed tomography (CT) with 68Ga-DOTA‐DPhe1, Tyr3‐octreotate (68Ga- DOTATATE) showed increased uptake in the right adrenal gland and a 11x10 mm nodular lesion in the corpus of the pancreas (Figure 2). The tumoral masses in the adrenal gland and pancreas were removed. Histologic investigation of adrenal and pancreas specimens confirmed the diagnosis of PCC and PNET (World Health Organisation grade 3) respectively. One year later, an 8x7 mm lesion in the pancreas, compatible with recurrence, was observed on abdominal MRI and confirmed with 68 Ga-DOTATATE PET-CT. Splenectomy and subtotal pancreatectomy were performed for removal of the lesion. Histologic examination of the pancreatic lesion reported a neuroendocrine tumor. Bilateral PCC with PNET suggested the diagnosis of VHL disease. Molecular genetic analysis of the VHL gene revealed a heterozygous missense mutation c. 386 T>G (p.Leu129Pro) which has been previously described. No additional VHL tumor developed during three years follow up.

    1. c.340G>A

Of 31 Brazilian patients, four presented with the c.340G>A (p.Gly114Ser) mutation of the VHL gene. Genomic DNA was extracted and all VHL coding regions were analyzed by Sanger sequencing. MLPA was performed to investigate large deletions using SALSA® MLPA® P016 VHL probe mix. Data analysis was performed with Genescan 3.7. Of these four patients, three were of the same family. Individual 1 of family 1, 38 years old, presented with paraganglioma, pheochromocytoma, and pancreatic endocrine tumor. Individual 2 of family 1, age 20, had a history of sudoresis, headaches, and abdominal pain. He was diagnosed with hypertension. At 4 years old, a CT scan revealed a 5cm heterogenous mass in the right adrenal gland and he underwent a right nodulectomy. 3 years later, a bilateral recurrence was found (4cm right and 1.5cm left nodules) and he underwent a right-side adrenalectomy and a left-side nodulectomy. 13 years later he presented with a left-side reoccurance (1.3cm nodule) and elevated plasmatic normetanephrine levels. He underwent left-side adrenalectomy. He also presented with paraganglioma, pancreatic endocrine tumor (at 20Y) and CNS hemangioblastoma. Individual 3 of family 1, 26 years old, presented with paraganglioma, pheochromocytoma, pancreatic endocrine tumor, renal cyst, CNS hemangioblastoma and retinal hemangioblastoma. The relation between these familials was not specified. Individual 4 of family 2 presented with left-side pheochromocytoma (6cm) and a right-side pheochromocytoma, (5.5cm) at 8 years. She also presented with pancreatic endocrine tumor (1.5cm) at 14Y.

    1. 2.3.1 DNA extraction Tumoral DNA was extracted from formalin-fixed, paraffin-embedded tissues as previously described [38]. Briefly, paraffin sections of tumor tissue were selected by a pathologist (MB) and tissue cores removed from the respective blocks to ensure that the tumor cells comprised more than 90% of the samples. Uninvolved normal tissue or peripheral blood (PB) was used as the source of normal DNA control for each patient for LOH analysis. Tumour tissue was digested with SDS/proteinase K for 24 h at 48 °C, and DNA was extracted with phenol/chloroform [38]. DNA concentration was quantified by absorbance measurement at 260 and 280 nm using the Nanodrop spectrophotometer. 2.3.2 Mutation analysis PCR amplification was performed searching for point mutation in the entire VHL coding sequence and promoter region, including exon-intron boundaries. Amplification was performed in a Gene Amp PCR System 9700 (Perkin Elmer, Foster City, CA) in a final reaction volume of 25 μl containing 100 ng of genomic DNA template, 0.25 nM dNTPs, 20 pmol of each primer, 1 U HotMaster Taq polymerase (Eppendorf s.r.l., Milan, Italy), in 1X PCR Reaction Buffer. PCR cycling conditions consisted of an initial denaturation step at 94 °C for 2 min, followed by 35 cycles at 94 °C for 30 s, the specific annealing temperature for 30 s, and ending with a final elongation step at 72 °C for 7 min. PCR products were purified using GFX™ PCR DNA and Gel Band Purification Kit (GE Healthcare, Buckinghamshire, UK) and sequenced. Sequencing reactions were performed in 10 μl of the final volume composed of 3 pmol of primer, 4–6 ng of DNA template and 1 μl of Big Dye Terminator Read Reaction mix v.1.1 (Applied Biosystems by Thermo Fisher Scientific, Waltham, MA USA). Sequencing reactions were loaded on an ABI 3100 capillary sequencer (Applied Biosystems) and analyzed using the Sequencing Analysis software v.3.7 (PE Applied Biosystems). When mutations were detected in DNA from tumor samples, we searched for the same mutation in the corresponding PB DNA in order to identify germline mutations. 2.3.3 Loss of heterozygosity analysis Loss of heterozygosity (LOH) analysis using fluorescent-labeled DNA primers from matched PB and tumor tissues was performed using three microsatellite markers flanking the VHL gene: D3S1335, D3S1038, and D3S1317. PCR was performed in a 50 μl reaction volume containing 2,5 μl of 10X PCR Buffer (Eppendorf), 0.25 nM dNTPs, 20 pmol of each primer, 1 U HotMaster Taq (Eppendorf) and 100 ng of DNA. Cycling conditions consisted of 40 cycles of LMS2 (ABI Prism™ Linkage Mapping Set, version 2) method performed on a GeneAmp PCR System 9700 (Applied Biosystems) for all markers. After capillary array electrophoresis (ABI 3100, Applied Biosystems), and data analysis by the ABI Genescan and Genotyper Software 3.7 (Applied Biosystems), LOH or allelic imbalance (AI) value was quantified using the formula <<(peak 1 height/peak2 height in tumor DNA)/(peak 1 height/peak 2 height in normal DNA)>>, and following stricter criteria than previously suggested [39]. In fact, the cut-off for defining an AI was lowered to 0.5 whereas an AI > 0.5 and <0.65 defined a borderline case. LOH status was confirmed at least twice for each marker. 2.3.4 Methylation analysis The methylation status of the promoter region of the VHL gene was determined by methylation specific PCR (MSP) as previously described [28]. Briefly, 1 μg of the genomic DNA was modified with EpiTect®Bisulfite Kit (Qiagen, Hilden, Germany). Modified DNA was amplified by specific primers for both unmethylated and methylated sequences. PCR was performed in 20 μl reaction volumes containing 1X PCR Buffer [16.6 mM ammonium sulfate, 67 mM Tris (pH 8.8), 6.7 mM magnesium chloride, 10 mM b-mercaptoethanol, 1% DMSO, and 1.25 mM deoxynucleotide triphosphates), 0.2 mM each deoxynucleoside triphosphate, 100 pmol of each primer, 2 units of PlatinumTaq polymerase (Invitrogen), and 50 ng of bisulfite-modified DNA. PCR cycling conditions were as follows: 2 min at 94 °C, 5 cycles of 30 s denaturation at 94 °C, 30 s at 64 °C and 30 s extension at 72 °C, 5 cycles at 94 °C, 30 s at 62 °C and 30 s extension at 72 °C, 25 cycles at 94 °C, 30 s at 60 °C and 30 s extension at 72 °C, and a final extension step of 7 min at 72 °C. CpGenome Universal Methylated DNA (Millipore, Merck Millipore Inc., Billerica, Massachusetts) was used as a positive control. PCR products were loaded on 3% agarose gels with ethidium bromide and visualized under UV illumination.

This study retrospectively reviewed the nephrectomy specimens for 20 patients, for whom histologic slides were available, from the authors respective institutes’ VHL clinics over a 15-year period (1996 to 2011). One patient (case 3) was found to have a history of VHL disease with a confirmed germline missense mutation (p.L128P (c.383T>C)) of the VHL gene. The 38 year old female proband presented with multiple bilateral renal tumors, and underwent multiple partial nephrectomies with tumor enucleations. Six of such tumors were identified and they ranged in size from 0.4-6.5 cm. Of these, 4 tumors were CCRCCs and 2 tumors showed were diagnosed as CCPRCCs confined to the kidney. Further, the tumors were classified as hemangioblastomas. ACMG evidence codes: 'PP4' since the patient presents phenotypes highly specific for a disease with a single genetic etiology.

    1. Case report A 6-year-old girl was brought into the ED by her parents in the late evening with a 5 week history of colicky, peri-umbilical pain and general malaise. Her mother reported that they had been to the family doctor repeatedly since the onset of symptoms and trialled a number of medications (including antispasmodics and various analgesics) without success. On further questioning, the patient described several episodes of spontaneous vomiting without obvious cause or association over the past month. Her appetite was poor and she had been losing weight. There were no other gastrointestinal or urinary symptoms. There was no history of fever, although she did report ‘sweating for no reason’ on occasion. She had also been suffering from intermittent, generalized headaches without any other focal symptoms over the same time period. She had been previously well, the only noteworthy point in her medical history being her conception through in vitro fertilization. There were no developmental concerns or family history of note. On examination, the patient appeared pale, tired, mildly dehydrated, but apyrexial and comfortable at rest. She was profoundly tachycardic, with a heart rate of 170 beats per minute, and had a BP of 203/157 mmHg on repeat measurements (normal systolic range 90–110 mmHg in this age group). Crucially, as the patient arrived in the early evening, she was triaged in the adult ED and not the paediatric ED (which was closed at this time), where BP would not have been part of routine observations in this age group. Urinary dipstick showed raised protein and blood. General examination was otherwise normal. In discussion with paediatric cardiology, the patient was started immediately on oral nifedipine to control the malignant hypertension prior to further investigation. Blood tests revealed low potassium of 3.0 mmol/L (normal 3.3–4.6) with raised urea (10.0 mmol/L; 2.5–6.5) and creatinine (81 mmol/L; 25–60). Routine haematology and biochemistry bloods (including liver and thyroid function tests) were otherwise unremarkable. An abdominal ultrasound scan demonstrated a 3 × 2.5 cm mass at the upper pole of the left kidney without a distinctly identifiable adrenal gland, a finding confirmed on CT (Fig. 1), which also showed some low attenuation suggestive of cystic change within the mass. Serial urinary metanephrines were subsequently found to be grossly elevated (17.0, 18.1 and 14.7 µmol/mmol creatinine; normal <1.74) with a predominant normetanephrine fraction, suggesting a diagnosis of phaeochromocytoma. Figure 1Open in figure viewerPowerPoint Ultrasound image (above) of the patient's left kidney (between the two crosses) clearly showing an abnormal mass associated with the organ. The adrenal cannot be distinctly identified. The mass was further characterized on CT (below, the mass having been circled). Sympathetic blockade was established first using gradually increasing doses of oral phenoxybenzamine (switched to i.v. dosing in the 5 days prior to surgery), then adding propranolol, aiming for a systolic BP of 80 mmHg with postural drop of 10 mmHg in order to facilitate safe removal of the tumour. Concurrent investigations identified raised serum renin and aldosterone levels consistent with secondary hyperaldosteronism. Serum calcitonin and urinary steroid profiles were normal, as was retinal screening for abnormalities associated with von Hippel-Lindau (vHL) disease, including retinal haemangiomas and hypertensive retinopathy. A laparoscopic left adrenalectomy took place 6 weeks after initial presentation, and priority was placed on close cardiovascular monitoring perioperatively. The patient was fit for discharge 3 days after surgery histology subsequently confirming the diagnosis of phaeochromocytoma.

Eligible patients were all index cases with clinical diagnosis or clinical suspicion of type 1 VHL referred for genetic testing from 2003 to 2015. Universal primer quantitative fluorescent multiplex polymerase chain reaction (UPQFM-PCR) and multiplex ligation-dependent probe amplification (MLPA) analysis was completed using peripheral blood leukocytes. This patient (ID #22) was found with this VHL nonsense mutation (c.264G>A) when they were 13Y. They did not fulfil type 1 VHL clinical diagnosis. They presented with CNS hemangioblastoma at 13Y. Family history was unremarkable.

    1. other VHL patients

An Israeli specialized center identified 32 VHL patients belonging to 16 diferent kindreds. This retorspective collection and analysis of clinical, labratory and imaging data of VHL patients involved one individual belonging to kindred II. Genetic analysis via direct VHL gene sequencing and quantitiative Southern blot identified the W88* (c.263G>A) mutation. This individual (49Y, M) had cerebellar hemangioblatomas, bilateral RCC and nephrectomy, as well as pancreatic surgery for his PNENs. He passed away at 54Y, without any family history reported.
