      The authors raise an important and intriguing finding that is the extraordinary response to Rituximab of some diseases. This fact is becoming an inteersting topic of research in the neuroimmunology field because several diseases show this type of response (Huijbers MG, 2015). Although the authors do not mention it, this finding may be related to the IgG4 nature of the pathogenic autoantibodies. This type of extraordinary and long-lasting response happens in several other diseases with very different target organs: myasthenia with anit-Musk antibodies (Díaz-Manera J, 2012), anti-contactin and anti-NF155 chronic inflammatory polyradiculoneuropathy (Querol L, 2015), anti-PLA2R nephropathy (Beck LH Jr, 2011)... Uncovering the differences that IgG4 plasma cells show with respect the more typical Ig1-3 would be key to understand the amazing response to rituximab in these diseases.
