      The paper by Dr. Macy in fact says: "Physician-documented cephalosporin-associated anaphylaxis was about 2.9-fold more common in individuals with histories of penicillin allergy than in individuals with no history of drug allergy." A remote risk of anaphylaxis supports using recommended first line antibiotics for cystitis such as nitrofurantoin or trimethoprim-sulfamethoxazole - see international guidelines (Clin Infect Dis. 2011;52: e103-e120) which add: "The β-lactams [such as cephalexin] generally have inferior efficacy and more adverse effects, compared with other UTI antimicrobials".
