    4. In a study of 114 unrelated VHL families, 85 germline mutations were found. VHL mutations were detected in affected family members, but not in unaffected family members or 96 normal individuals. This missense mutation was found in 3 VHL type 1 family members (family ID 3738), using PCR and SSCP sequencing. The mutation of c.334T>C was referred to in the paper by c.547T>C due to a difference in transcript sequence. All had pheochromocytomas and one with retinal hemangioblastomas.

216 patients with clinically expected VHL disease due to family history or presence of VHL typical tumours were routinely examined in an eye centre in Germany between January 2019 and January 2020, majority of which were of European descent (~95%). Since data on the genotype–phenotype correlation of the onset of Retinal Hemangioblastoma (RH) is scarce in relation to VHL disease, this study wanted to inversitage the relationship between VHL gene variants (truncating vs single amino acid substitutions) and the ocular phenotype of the disease. 42 different rare VHL gene variants in 148 patients were detected - some of whom had RH and some who did not. Although date of onset of RH, as well as data on central nervous system haemangioblastomas (CNSH), pheochromocytoma/paraganglioma (PPGL), clear cell renalcell carcinoma (ccRCC) and pancreatic neuroendocrine neoplasm (PNEN) were collected, it was not reported on a per patient basis and therefore could not be discussed per variant. There was 1 patient reported with the c.386T>C (p.L129P) mutation. This member presented with retinal hemangiomas. No family information was provided.

    1. c.A233G (p.N78S)

Two VHL syndrome pedigrees were investigated in this study; family 1 was a three-generation pedigree including 6 individuals. In family 1, the proband (F1-II-1), a 31-year-old male patient, presented with headache, paroxysmal hypertension and dizzy spells in 2009 with a 4-year history. Ultrasound and MRI scans confirmed single masses, multiple tumors and multiple cysts in the right adrenal glands, bilateral kidney and pancreas, respectively. Right pheochromocytoma and multiple right renal cell carcinoma were confirmed by histopathological examination. In 2012, the patient was diagnosed with bilateral epididymal head multiple nodules. Bilateral epididymal tubercle resection was performed and epididymal papillary adenoma was confirmed by histopathological examination. Additionally, the patient's younger brother (F1-II-3, 30 years old) presented with multiple right renal masses, epididymal nodules and multiple pancreatic cysts. Radical right nephrectomy was performed and histopathological analysis revealed right RCC with hemorrhagic cyst. In family 1 they identified a heterozygous variant, c.233A>G (p.N78S), within exon 1 of the VHL gene in two affected cases and one clinically normal family member (F1-III-1, the proband's 5-year-old son). Peripheral blood samples were collected for mutation analysis of the RET proto-oncogene and VHL tumor suppressor gene. PCR amplification of all exons and the flanking splice junctions of RET and VHL was performed, followed by direct bidirectional DNA sequencing. One hundred unrelated controls with a similar ethnic background were also included in this study. ACMG evidence codes: ‘PP4’ because the family history is highly specific for a disease with a single genetic etiology, ‘PP1’ because there is co-segregation of the variant with VHL disease in the family.

A total of 291 patients from 115 different families were included for analysis. One male patient was found with this germline mutation from family #45. He was diagnosed with CNS hemangioblastoma at 30Y. One male patient was found with this germline mutation from family #46. He was diagnosed with CNS hemangioblastoma at 30Y, RCC at 31Y, pheochromocytoma at 30Y, and multiple pancreatic lesions at 30Y. No other family information was published for these two patients. Both kindreds were also likely published in a paper by Wang et al., 2018 (PMID: 29330336). Kindred #46 was also previously published in Wang et al., 2014 (PMID: 24581539). Wang et al., 2018 reported 4 patients from 2 families (corresponding with kindred #45 and #46) who were genetically confirmed with this germline mutation. Wang et al., 2014 reported one patient who was genetically confirmed with this germline mutation likely corresponding with family #46.

    3. A retrospective cohort study included all the VHL patients diagnosed at Peking University First Hospital (Beijing, China) prior to June 1 2016. A total of 291 patients from 115 different families were included for analysis. Genotyping confirmation was performed by PCR-direct sequencing on peripheral blood samples. This mutation was confirmed in families #4, #5, and #5. Only the proband of family #4 (male, patient #7) was genetically confirmed with the mutation. He was diagnosed with renal cell carcinoma and multiple pancreatic cysts/tumors at age 33. Only the proband of family #5 was genetically confirmed with the mutation. He presented with a CNS hemangioblastoma at age 21 and multiple pancreatic cysts and tumors at age 30. His grandfather, father, and uncle presented with a CNS hemangioblastoma at ages 30, 36, and 33, respectively, and all passed away before they could be diagnosed with VHL disease. Family #6 is large: the proband (F, renal cell carcinoma at age 36 and pheochromocytoma at age 37) had 8 family members genetically confirmed with the mutation and two family members who passed away before diagnosis. The 8 genetically confirmed family members include patient 13 (asymptomatic,) 14 (CNS hemangioblastoma at age 59,) 15 (CNS hemangioblastoma age 45, renal cell carcinoma at 59, multiple pancreatic cysts/tumors at 65), 18 (CNS hemangioblastoma at age 22, renal cell carcinoma at 30, multiple pancreatic cysts/tumors at 31,) 19 (asymptomatic), 20 (asymptomatic), 21 (CNS hemangioblastoma and retinal angioma at 53,) and 22 (multiple pancreatic cysts/tumors at 29). The 2 family members who passed away before diagnosis are patients 16 (CNS hemangioblastoma at 22) and 17 (CNS hemangioblastoma, renal cell carcinoma, and multiple pancreatic cysts/tumors at age 51). This cohort was also published in a paper by Wang et al., 2018 (PMID: 29330336).

    4. Materials and methodsWe considered an individual to have a confirmed diagnosis of vHL if they met the clinical criteria for vHL (box 1) and/or they tested positive for a disease-causing variant in VHL. We defined vHL families as families in which two or more individuals had a confirmed diagnosis of vHL. Using these criteria, we identified 27 vHL families by retrospectively analysing data that had been entered between 30 August 1984 and 8 August 2016 in our SHIRE genetics database and SickKids Genetics and Cancer Genetics Program records. Of the 27 families identified as being tested for vHL, 7 were excluded as either having no information on family history or for de novo cases. Only those members of vHL families for whom there was sufficient information to confirm both a diagnosis of familial vHL and AOO of first vHL-related clinical manifestations were included in the study. A total of 96 patients (n=43 men, n=53 women) from 20 vHL families met these criteria. Both clinically affected (n=80) and clinically unaffected (n=16) were included in the analysis. The cohort was ethnically diverse, with ancestors from Africa, Asia, Europe, North America and South America. The year of birth, type of vHL-related manifestation, year at which the manifestation was diagnosed and year last ascertained (the last reported contact with the patient or family by healthcare providers at SickKids) were noted for each patient. Telomere lengths were collected for patients after 2014 as part of their clinical assessment and were also included in this retrospective health records review when available.

    1. VHL sequencing. A missense mutation (c.481C>T) causing a premature stop codon (p.Arg161*) and a truncated VHL protein (black box, c.481 location; nucleotide C (blue line) was changed to T (red line) in heterozygosity. VHL, von Hippel-Lindau.

A 19-year-old female patient was referred to a psychiatric inpatient service due to a suspicion of anorexia nervosa (AN) following severe weight loss, low food intake, and a lack of a menstrual cycle for at least 1 year. The patient is the second child of a non-consanguineous relationship and there were no abnormalities in developmental milestones, but her weight never exceeded 35 kg. Apart from weight loss and weakness, physical and neurological examinations showed no alterations. The patient frequently complained of epigastric burning sensation, nocturnal headaches, postural hypotension and difficulty swallowing. The patient did not meet the necessary criteria for a diagnosis of AN or another psychiatric disorder. The patient experienced episodes of nausea during fasting after waking up, followed by unprovoked vomiting, which is highly uncommon in AN. Blood tests, urinalysis and chest X-ray were all normal. Abdominal CT scan revealed cystic lesions throughout the parenchymatous portion of the pancreas, suggestive of VHL. After 3 weeks of hospitalization, the patient progressively developed gait abnormality and bilateral dysmetria, which was worse on the left side. A brain MRI examination revealed a massive lesion suggestive of a hemangioblastoma (HB) in the fourth ventricle causing hypertensive hydrocephalus. HB and pancreatic cysts support the diagnosis for VHL. Whole blood was obtained for sanger sequencing of all 3 exons of the VHL gene, revealing a nonsense mutation in the VHL gene (c.481C>T; p.Arg161*) that results in a truncated protein. In silico analyses by mutation analyzer 2.0.28 suggests a pathogenic variant. Surgery inserting a ventriculoperitoneal shunt to relieve the hypertensive hydrocephalus only temporarily relieved her appetite; yet she no longer had fasting nausea and vomiting, or nocturnal headaches. Fractionated radiotherapy did not improve appetite or nausea but was successful in controlling tumor growth as an MRI revealed no significant alterations in the size of the lesion six months post-radiotherapy.

    1. c.371C>Tp.Thr124Ile

Genomic DNA was extracted from whole blood samples for genetic testing of SDHB, SDHC, SDHD, and VHL. 27 patients with urinary bladder paragangliomas, revealed 17 germline mutations, three of which were in the VHL gene. PCR–based bidirectional sequencing was performed along with MLPA on large deletions. This missense mutation (p.T124I) was found in a 12-year-old male (patient no. 22), with his urinary bladder paraganglioma diagnosis at 6Y of age. Family history of pheo or paragangliomas was said to be positive.

    1. Medical records of 26 patients with germline mutations of the VHL gene who had been diagnosed with VHL disease in Severance Hospital (Seoul, Republic of Korea) and Samsung Medical Center (Seoul, Republic of Korea) between 2003 and 2012 were respectively reviewed. Peripheral blood leukocytes were isolated and directly sequenced using the BigDye Terminator Cycle Sequencing Ready Reaction kit on an ABI Prism 3130 genetic analyzer (Applied Biosystems). One 34Y male patient (Patient 15) presented with multiple and/or bilateral RCC and a CNS hemangioblastoma. He had a germline missense mutation in the VHL gene (c.233A>G p.Asn 78 Ser). The patient had a negative family history of VHL disease. The phenotypes of this patient were revised from a previous report in PMID 19270817 and/or 19949673.

    1. Patient II‐1 was admitted to the Department of Urinary Surgery and Neurosurgery in October 2007 and again in December 2015. In the first hospitalization, a serendipitous left kidney mass was found, but no symptoms. Abdominal CT revealed a solid cystic mass on the inside of the left kidney with irregular enhancement (Figure (Figure2b),2b), indicating a radiological diagnosis of RCCC. A laparoscopic radical nephrectomy was performed, and pathology confirmed the diagnosis of clear cell carcinoma. In the second hospitalization, the patient was admitted with complaints of headache and dizziness for 2 months. A brain MRI showed a right CH (Figure (Figure2c).2c). Tumor resection was performed through a posterior approach, and pathology confirmed the diagnosis of hemangioblastoma. In the most recent follow‐up, his symptoms had disappeared, and radiological imaging revealed no recurrence in either the brain or abdomen.

In a study of 138 unrelated VHL families, 109 germline mutations were found. Missense mutations were more frequently found in families with pheochromocytoma (PC). Deletions. Nonsense, and frameshift mutations were more frequently found in families without PC. Large deletions and mutations predicted to cause truncated protein were associated with lower risk of PC. The mutation of c.214delT was referred to in the paper by c.427delT due to a difference in transcript sequence. This deletion mutation was found in a VHL family without PC (family ID 112). 1. Result: 30 year old female presented with headache, palpitation, epistaxsis for 2 months. On examination: Bp 210/110 mmHg, physical examination unremarkable. On Investigation: Ultrasound revealed a Rt adrenal mass and a pancreatic mass, which was confirmed on a CECT abdomen pelvis. A CECT brain revealed a cerebellar haemangioblastoma. With these findings diagnosis of probable VHL made. Patient then underwent molecular genetic testing and was found to have heterozygous mutation in C293A. C in the VHL gene. Metabolic workup and Bp control was done by endocrinology After adequate BP control patient posted for adrenalectomy. Patient underwent an uneventful adrenalectomy. On HPE the diagnosis of adrenal pheochromocytoma was confirmed. Department of neurosurgery performed surgery for cebellar haemangioblastoma and in the GI surgery department were she underwent a distal pancreaticospleenectoy. Patient was then desirous of pregnancy. Department of OBG counselled her and she had a planned conception. She was managed for severe pre eclempsia, pregnancy in a fibroid uterus by OBG and endocrinology, and she delivered a healthy chid. Patient is presently a controlled hypertensive and is on regular follow up.

An Israeli specialized center identified 32 VHL patients belonging to 16 diferent kindreds. This retorspective collection and analysis of clinical, laboratory and imaging data of VHL patients involved one, 3 generation kindred identified and reported as early as 1980. Genetic analysis via direct VHL gene sequencing and quantitiative Southern blot identified the V166F (c.496G>T) mutation. This kindred (family 1) most recently have a reported 14/32 total patients diagnosed with VHL. One of which (I-31) was a mutational carrier with down syndrome. The other 13 patients had varying phenotypes, including adrenal pheochromocytoma, pancreatic endocrine tumors, liver and bone metastases, paraganglioma, cerebellar, spinal, and retinal hemangioblastomas, polycythemia, and chondrosarcoma. See Fig. 1 for family pedigree. Note: This kindred was previously published in PMID: 8550742. Since its initial publication in 1996, several asymptomatic carriers developed a neuroendocrine tumor, 7 new children were born where 3 of them carry the mutation. In addition, 2 patients died, one of which was due to metastatic PNEN-related liver failure.

27 index patients suspected of having VHL disease from unrelated Chinese families and their relatives were studied. The nucleotide were numbered according to the sequence numbering by Latif et al, 1993 and this was converted to NM_000551.3. The paper did not differentiate between a pancreatic cysts and tumor and presented these together as a 'pancreatic lesion'. The paper summarized the phenotypes of the families together. Family 9 and 10 were found with p.Pro86Leu mutation in the VHL gene. Family 9: the mutation was detected in a 53 years old woman (proband) with asynchronous bilateral renal clear cell carcinomas, multiple renal cysts and pancreatic cysts, an asympomatic member and with (initially) no positive family history. Subsequently, genetic analysis and clinical evaluations showed that the same mutation was also detected in her niece who had no detectable VHL lesions and in her young brother who had pancreatic cysts. Family 10: 1/4 members had CNS hemangioblastoma, 1/4 had retinal angioma, 2/4 had RCC, and 1/4 had a pancreatic lesion. ACMG PP4

    2. Genotype-phenotype correlations of 573 VHL patients from 200 kindreds were analyzed. Higher risk of pheochromocytoma was associated with missense mutations that result in substitution of a surface amino acid (p<=0.004) than for patients with missense mutations that disrupt the structural integrity of the VHL gene product (pVHL) or other loss of function mutations (either germline deletions or truncating mutations). A missense mutation, S65W(c.194C>G), affecting residue within the pVHL protein core, was found in a VHL family with 8 affected individuals including 6 with retinal hemangioblastomas, 5 with cerebellar hemangioblastomas, and 1 with renal cell carcinoma. ACMG codes as follows: supporting evidence for pathogenicity because of cosegregation of disease with multiple affected family members in a gene definitively known to cause the disease (ACMG code: PP1).

    1. PatientsThe Chinese family study included five-generations comprising of 38 living family members, of whom nine were affected [Figure 1]. All living members were examined clinically. The clinical and radiographic images were published with the consent of the patient. The study was approved by the research Ethics Committees of Xi’an Jiaotong University School of Medicine. Written informed consents were obtained from all participants.Open in a separate windowFigure 1Haplotypes of the Chinese family affected by Von Hippel-Lindau (VHL). The open and filled blackened symbols indicated unaffected and affected individuals, respectively. The squares and circles symbolized the males and females, respectively. The proband, IV-3, was indicated by an arrow. The haplotyping was conducted using three polymorphic microsatellite markers. The marker order was determined from the Marshfield map and the UCSC Human Genome database (February 2009). The dark boxes symbolized haplotypes that cosegregated with the affected individuals and suggested linkage of VHL to disease in this family. The boxes with slash symbolized the dead family members.

Of 38 PPGL (pheochromocytoma and paraganglioma) cases studied, three were associated with an individual carrying the germline mutation c.481C>T (ENSP00000256474.2 p.Arg161*) of the VHL gene. This mutation was identified using germline clinical sequencing on formalin-fixed and paraffin-embedded (FFPE) tissue and confirmed with sanger sequencing. The authors identified the reference sequence used as NM_000551.3. The first patient (ID: RNSH06) presented with bilateral adrenal phechromocytoma (34yo), ID: RNSH09 with right adrenal phechromocytoma (39yo), and ID: RNSH10 with left adrenal phechromocytoma (40yo). No family history, or relation, was presented. RNSH09 and RNSH10 were likely previously published in PMID: 25545346.

53 patients with a molecularly confirmed VHL mutation (by PCR through EDTA-anticoagulated blood) and a pancreatic neuroendocrine tumor (ICT) were collected from the German NET-Registry and the German VHL-registry. An additional 296 patients were stated in this paper with a VHL associated germline mutation, but without pancreatic neuroendocrine tumors. 13 patients were found to have the above mutation (c.482G>A). 5 were VHL patients with ICTs (pancreatic neuroendocrine tumors) taken from the VHL-registry. The other 8 patients were stated to be in this study with the given mutation, however no ICT. Other clinical manifestations outside of a pancreatic neuroendocrine tumor are possible, but were not discussed. ACMG evidence codes: 'PP2' because they observe a missense variant in a gene where missense variants are a common mechanism of disease.

    1. In the present study, we have analyzed 135 apparently sporadic patients developing a single tumor for the five major susceptibility genes: VHL, RET, SDHB, SDHC, and SDHD. Fourteen percent of cases were found to harbor a germline mutation, and only 2.2% of patients were older than 45 years at onset. By taking into account the tumor location and a threshold age at onset of 45 years, we propose a rational scheme for genetic testing.

ArticleReferenceSequence: Not specified – Assumption: variants assumed to have been documented by the author under reference sequence NM_000551.3

GenotypingMethod: Complete genetic characterization of the five susceptibility genes mentioned in this paper included point mutation analysis of all exons and intron-exon boundaries. In order to assess the meaning of undescribed variants, the authors used DNA from 1000 unrelated and unaffected individuals as a control population.

SamplingMethod: Unspecified. According to the paper, DNA samples were obtained by "standard methodological procedures" as stated in Sambrook et al., 1989: Molecular Cloning: A Laboratory Manual. 2nd ed. Paper indicates germline genetic analysis was performed.

    1. c.269A>T p.Asn90Ile

During the period from 2009 to 2012, 39 families with VHL disease were diagnosed with hereditary VHL disease at Peking University First Hospital Department of Urology were described as part of a broader study examining telomere length and anticipation. In these families, 19 families had two or more than two patients in two or more than two generations, including one family with obscure onset age. Therefore, a total of 57 patients with VHL disease from 18 families were enrolled in this study. Using peripheral blood and southern blotting, DNA sequencing was completed on the following patients. Family 4 involved three affected members, however only two of which were known to have a c.269A>T change leading to a p.Asn90Ile protein change. The male proband presented with hemangioblastoma at 22 and retinal capillary hemangioma at 32. His mother presented with renal cell carcinoma at 63. The proband’s brother died before diagnosis; thus no DNA sample available, however he was affected with hemangioblastoma at age 19Y. Of family 9, the male proband presented with hemangioblastoma at 28, renal cell carcinoma at 38, and pancreatic cysts at 38. His mother, aunt and cousin all died before diagnosis; thus no DNA sample available. The mother was last observed at 33Y with CNS hemangioblastoma; the aunt at 32Y with hemangioblastoma, and the cousin at 30Y with hemangioblastoma. ACMG Evidence Code: 'PP4' because the patients' phenotypes and family histories are highly specific for VHL disease.

    2. Two unrelated patients were screened for VHL mutations using SSCP and sequence analysis. Patient W was found with a germline splice site variant mutation c.464-1G>A (stated as 677-1G>A in the paper which used the Latif sequence as reference). They had no prior family history but parents were not available for genetic testing. The patient had hemangioblastoma and renal cell carcinoma.

    1. Familiar pedigree of the proband (arrow) with von Hippel-Lindau syndrome.

A 49-year-old woman had episodes of vertigo 12 years ago and abdominal pain 10 years ago. Ultrasonography and abdominal MRI revealed the presence of pancreatic cysts. Seven years ago, an abdominal computed tomography scan showed a solid mass in the right kidney. A right partial nephrectomy was performed and pathological examination revealed renal cell carcinoma of the right kidney. That same year, the patient complained of altered visual acuity and a CT scan of the skull and orbit revealed thickening of the left optic nerve and the right optic nerve had a conserved aspect. These results were suggestive of a hemangioblastoma (Hb) in the optic nerve of the left eye. Peripheral blood was sampled to extract genomic DNA for VHL gene sequencing and the detection of VHL copy number variants using multiplex ligation-dependent probe amplification (MPLA). A heterozygous W88* (c.263G>A) nonsense variant was identified in the VHL gene. The patient's family history revealed siblings with VHL manifestations. The patient's mother was the index case and died at age 63 of clear cell renal carcinoma. The patient has one child, who is 27-years-old and complains of recurrent headaches, but refuses to undergo VHL screening. The patient's 5-year-old grandson does not have symptoms. The patient has five brothers and two sisters. One sister underwent partial nephrectomy due to carcinoma in the right kidney, another sister was operated on due to CNS Hb and had pancreatic cysts, and one brother had two Hbs removed that were detected in the brain and spine. One of the patient's nephews was diagnosed with a CNS Hb at age 12 and died 4 years later due to complications with the brain tumor that was not removed. Another niece was diagnosed at age 21 of cerebral Hb, and had the tumor resected. Upon follow-up, the patient still possesses pancreatic and renal cysts, but without renal tumor recurrence or metastases. The right optic nerve is intact, and there are no CNS lesions. She has slight decrease in visual acuity in the left eye due to the optic nerve Hb. Genetic testing of any of the patient's family members was not reported.

    1. Of 65 VHL families from central Europe, 53 were identified with germline mutations using SSCP analysis of PCR products by peripheral blood. This missense mutation was found in a German, VHL type 1 family of 2. Both patients had hemangioblastomas of the central nervous system, renal cysts, pancreatic cysts, and cystadenomas of the epididymis (family VHL 60). Also, 1/2 patients had retinal angioma, renal cell carcinoma and liver angiomas. The H115R (c.344A>G) mutation was detected in the patient, referred to in the paper by c.557A>G due to a difference in transcript sequence. This study contains supporting evidence of pathogenicity due to cosegregation with disease in multiple affected family members in a gene definitively known to cause the disease (ACMG code: PP1).

    2. Of 65 VHL families from central Europe, 53 were identified with germline mutations using SSCP analysis of PCR products by peripheral blood. This splice mutation was found in a Italian, VHL type 1 family. Only one affected patient was identified with retinal angiomas, hemangioblastomas of the central nervous system, renal cell carcinoma, pancreatic cysts and cystadenomas of the epididymis. (family VHL 31). The c.464-2A>G mutation was detected in the patient, referred to in the paper by c.677-2A>G due to a difference in transcript sequence.

    1. Germline mutations in 45 unrelated Japanese VHL patients were determined from blood samples using PCR-SSCP or Southern blot analyses. The authors reported a nucleotide deletion (i.e. 683C>T) which coincides with legacy variant nomenclature (AF010238.1) and was mapped to c.470C>T (NM_000551.3). PCR-SSCP or Southern blot analyses of the DNA from family members showed that mutations were segregated with disease phenotype in VHL families, although this data was not shown. Data on the presence of retinal angioma, CNS hemangioblastoma, renal cell carcinoma (RCC), and pheochromocytoma was available. Case 45 had a p.Thr157Ile (c.470C>T) mutation. 2 members were affected: one had RCC and the other had RCC and pheochromocytoma. Other demographics of the members were not described.

The patient's previous medical history included urological surgery at the age of 23 years due to an epididymal cyst and an ophthalmologic surgery at 26 years of age for retinal detachment resulting from bilateral retinal angiomatosis. Contrast-enhanced computed tomography (CT) scanning and angiography were used to detect the presence of quasi-circular enhancing lesions in the middle of the left kidney and large irregular enhancing lesions in the right kidney, extending into the vena cava, that formed an embolus. In addition, multiple cysts in the pancreas and kidney were detected radiologically. The patient underwent a radical nephrectomy with an embolectomy of the right kidney for treatment of T3b-stage renal cell carcinoma. After three months, he successfully underwent laparoscopic nephron-sparing surgery (NSS) of the left kidney. Histological analysis of the renal tissue revealed clear cell carcinomas.

    1. In the analysis of 63 VHL patients, 55 patients were found with pancreatic involvement. Genetic tests on 35 of 55 VHL patients revealed VHL mutations in 28 patients. Hemangioblastomas of the central nervous system were found in 47 patients, renal cell carcinoma in 25, and pheochromocytoma in 11 patients. Peripheral blood samples were extracted and PCR sequencing was used to identify point mutations and multiplex ligation-dependent probe amplification (MLPA, P016-C2, MRC-Holland, Amsterdam, Netherlands) analysis was used to detect large rearrangements of the VHL gene. This missense mutation, c.256C>T, was found in a 26-year-old female with VHL type 2B phenotype (case no. 6). This patient presented with serous cystadenoma of the pancreas and pancreatic endocrine tumor. The patient was followed up after 35.8 months. The progression of VHL disease continued even after surgical resection.

    2. In the analysis of 63 clinically diagnosed VHL patients, 55 patients were found with pancreatic involvement. Genetic tests on 35 of 55 VHL patients revealed VHL variants in 28 patients. Of the 55 enrolled patients, hemangioblastomas of the central nervous system were previously documented in 47 patients, renal cell carcinoma in 25, and pheochromocytoma in 11 patients. This missense mutation, c.208G>A, was found in 37-year-old female (case no. 1) who presented with pancreatic endocrine tumour and underwent a pylorus-preserving pancreaticoduodenectomy. The patient was followed up 27.7 months later.

    1. (c.695 G>A)

A case report of a 16-year-old male with VHL is described. This patient presented at the age of 10 with complaints of headache, diplopia and was found to have intractable hypertension and bilateral pheochromocytoma as well as developing bilateral retinal hemangioblastoma at age 16. Family history revealed the proband’s mother had pheochromocytoma at age 23. These findings taken together prompted genetic screening for VHL. The above mutation was found in both the proband and mother. The variant was reported as “c.695G>A” which was mapped to p.Arg161Gln (c.482G>A) in accordance with reference sequence NM_000551.3.

Two patients and their families were presented in this paper with PKD1 and PKD2 mutations. Case 2, with 3 familial generations ADPKD, was diagnosed soon after birth with multiple small cortical cysts in both kidneys. The 4 year old male inherited a PKD1 and PKD2 mutation from each parent: c.7288C>T p.(Arg2430), and c.124_125ins52bp p.(Ala42fs66). A heterozygous variant in the VHL gene (NM_000551.3 (c.556G>A p.[Glu186Lys]) was additionally identified in this individual through a gene panel based Next Generation Sequencing approach. Patient is an offspring of a family with 3 generations of autosomal dominant polycystic kidney disease (ADPKD). Father had mild liver cysts and found to be a carrier of the truncating mutation in PKD1 at 18Y0M. Mother had mildly enlarged kidneys with small cysts bilaterally, as well as liver cysts and found to be a carrier of a truncating mutation in PKD2 at 19Y0M. No information provided on whether the parents possess the VHL mutation.

    1. A149S (34)

301 were enrolled in a prospective clinical protocol to evaluate the natural history of vHL-associated pancreatic lesions (NCT00062166). Of these, 69 patients with pancreatic manifestations of vHL disease and missense VHL mutations were included in the current study. Individual patient phenotype was not described, but overall of the 60 patients, 57 had RCC, 41 had CNS hemangioblastomas, 28 had retinal hemangioblastoma, 23 had pheochromocytoma. 36/69 patients were female and the mean age was 50.5 years old.All patients underwent genetic testing using Sanger sequencing and Southern Blotting. Four patients were listed with this missense mutation, c.445G>T.

17 patients from 10 families affected with VHL syndrome, particularly with retinal capillary hemangioblastomas (RCHs) were examined. Details about this patient are not specified in the study, but patient 15 from family 9 was found to have several tumours, including CNS hemangioblastoma. Sanger sequencing of a whole blood sample revealed a germline S65W(c.194C>G) missense variant in exon 1 of the VHL gene. Patient 15 has a family member (patient 16) who also has several tumors and the same germline S65W(c.194C>G) mutation, but the familial relationship to patient 15 is not noted. Neither patient had RCH.

Tissue specimens of apparent sporadic pheochromocytoma were retrieved from the archives of the Department of Pathology of the Erasmus MC (Rotterdam), the Radboud University Nijmegen Medical Center, and the University Hospital Zurich (Switzerland). Mutation analysis was performed using SSCP analysis and BigDye® Terminator V3.1 cycle sequencing kit (Applied Biosystems, Foster City, CA). 136 VHL cases and six germline VHL mutations were found. A c.440T>C, p.Ile147Thr mutation (stated as c.653T>C in the paper which used the Latif sequence as reference) was found in one 58Y female patient with benign adrenal pheochromocytoma. No other phenotypes, patient demographics, or family history was described.

    1. The authors described the growth kinetics of 96 renal tumours in 64 VHL patients (32 males, 32 females). All patients had VHL disease with renal involvement. 54/64 had undergone renal surgery or a radiofrequency thermal ablation procedure for RCC. In 10/64 patients, at least one or more RCC was diagnosed. However, at the time of diagnosis, an indication for treatment was not yet present. For all patients, several CT/MRI scans existed for retrospective description of tumour growth kinetics. 10-ml EDTA anticoagulated blood samples were available from each patient for mutation screening of all exons of the VHL gene. Genomic DNA from blood samples was extracted using standard methods. Small deletions/insertions or point mutations were identified by DNA sequencing followed by complete mutation screening using PCR/single-strand conformation polymorphism and denaturing high-performance liquid chromatography. A germline missense variant p.V130F(c.388G>T) was identified in two patients: patient #7 and patient #10. This mutation was noted as c.601G>T in the paper due to a difference in reference transcript used (see Table S1). No family information is available for either patient, and there is no indication these patients are related.

    1. Peripheral blood from unrelated patients with pheochromocytoma was tested for mutations of proto-oncogene RET, tumor suppressor gene VHL, succinate dehydrogenase subunit D (SDHD) gene, and the succinate dehydrogenase subunit B (SDHB) gene. Of 271 patients with nonsydromic pheochromocytoma and no family history of the disease, 30 mutations were found in VHL. Multiple pheochromocytomas were statistically more frequent among patients with mutations than among patients without mutations (21 of 66 vs. 5 of 205, P<0.001). The mutation R167W (c.499C>T) was detected in the patient, referred to in the paper by c.712C>T due to a difference in transcript sequence. This missense mutation was found in the VHL gene and six patients with pheochromocytoma. Any relation is unstated. This mutation was said to be spontaneous.

    1. Supplemental Table S1

Six patients with classic VHL disease (VHL patients) and 6 healthy control participants took part in the study. The patients were free from any associated neoplasia at the time of the study and had no other medical disorders. With respect to the VHL patients, the aim of the authors was to determine whether any aspects of the CP phenotype are detectable in classic VHL disease. One 42Y male VHL patient with phenotype Type 2C VHl (pheochromocytoma only) was described to have the Pro81Leu VHL mutation.

    1. c.499C>Tp.R167W

Medical records of patients with PPGLs between 2005 and 2016 were reviewed. Diagnosis was confirmed histologically and stratified into sporadic or familial/syndromic. Patients underwent next‐generation whole-exome sequencing. A 17-year-old patient presented with unifocal (adrenal) pheochromocytoma. Genetic testing confirmed a familial missense mutation at c.499C>T of the VHL gene.

    1. Molecular and clinical analysis of 126 French VHL families tested for renal involvement revealed 92 different mutations. 90 of these families had renal involvement. The p.C162W (c.486C>G) germline variant of the VHL gene was detected in 3 individuals (ID 1114/Family 236, ID452/Family 345 and ID 9035/Family 379) who all presented with renal cell carcinoma. The paper states that these individuals were all previously reported in PMID 12202531 and/or 9829912.

    2. Molecular and clinical analysis of 126 French VHL families tested for renal involvement revealed 92 different mutations. 90 of these families had renal involvement. The p.Q73* (c.217C>T) germline variant of the VHL gene was detected in a family (ID 64). Members 2339, 2333 and 2338 presented with severe renal disease (RCC). Members 2331, 2329 and 1345 had unknown renal involvement. Member 2340 had mild renal disease. Member 2341 had no renal involvement. The paper states that this family is previously reported in PMID 12202531, where 4 patients with this mutation are reported but relation between patients is unknown. This variant was also found in another family (ID 84). Member 4605 presented with mild renal disease, member 4580 presented with moderate renal disease (RCC) and members 4581 and 4603 had unknown renal involvement. This family was not previously reported.

    1. Arg167Trp (712C-T)

Genomic DNA was extracted from peripheral blood and PCR amplifications of the three exon regions were performed. 4 of 5 families presenting with non-syndromic familial pheochromocytomas were found to have VHL germline mutations. The mutation R167W (referred to in the paper by c.712C>T due to a difference in transcript sequence) of the VHL gene was found in one proband and was confirmed to occur in other members of their family. The patient and their familial members had no clinical manifestation of hereditary syndromes, including MEN 2A, MEN 2B, VHL disease, and NF1. The laboratory and image examinations also excluded the presence of the above-mentioned hereditary syndromes. ACMG evidence codes: 'PP4' since the patient presents phenotypes highly specific for a disease with a single genetic etiology.

    1. This case report documented the promising short-term radiological response to somatostatin analog therapy of multiple nonmetastatic, nonfunctional PNETs in a patient with VHL. A 24 year old woman was confirmed with a germline VHL mutation (c.499C>T). She was diagnosed with bilateral pheochromocytomas and a pancreatic neuroendocrine tumour. Her aunt also had a pancreatic neuroendocrine tumour, but was not confirmed with the variation.

    1. VHL (NM_0000551.3: c.593 T > C; p.Leu198Pro)

A 55-year-old male presented to the emergency room due to shortness of breath, chest pain, and coughing. A chest X-ray and CT scan found a perihilar mass on the left lung, as well as a left renal lesion. A CT scan of the abdomen and pelvis revealed a mass on the left kidney and right adrenal gland. A biopsy showed that the left kidney mass was a renal cell carcinoma. The patient had a left-sided radical nephrectomy and right-sided adrenalectomy. Pathology confirmed clear cell renal cell carcinoma (ccRCC) and pheochromocytoma. Two pieces of lung tissue were orally expelled and examination supported metastatic ccRCC. Spinal MRI and chest CT identified both bone metastases and two spinal lesions consistent with spinal hemangioblastomas. Histology also confirmed liver metastases from the ccRCC. 10 months after his initial presentation, the patient succumbed to his disease 10 months. The patient had no family history of VHL and no other VHL-associated manifestations. The patient was clinical diagnosis with Type 2B VHL. Germline DNA from peripheral blood leukocytes was isolated and sent for gene panel genetic testing targeting known hereditary renal cell cancer and hereditary paraganglioma-pheochromocytoma genes. The gene panel targeted FH, FLCN, MET, MITF, MLH1, MSH2, MSH6, PMS2, PTEN, SDHA, SDHB, SDHC, SDHD, TP53, TSC1, TSC2, MAX, MEN1, NF1, RET, SDHAF2, TMEM127, and VHL. The patient tested negative for both single nucleotide or copy number variants in any of the genes. The variant allele threshold for this germline panel was >10%. Subsequent sequencing analysis of all four postmortem tumor tissues revealed an L198P(c.593T>C) missense VHL mutation at varying allele fractions (range: 10–55%) in all four tissues. Both of the patient's daughters underwent germline genetic analysis of VHL and were negative. However, due to family history, both daughters underwent full VHL screening including ophthalmologic assessment, brain, spine, and abdominal MRI. They have no VHL manifestations at the ages of 25 and 30. The germline genetic testing results of the proband's blood sample were re-analyzed and the VHL missense variant was detected at 6% variant allele frequency (below the standard 10% threshold), strongly suggesting that this case is a result of somatic mosaicism.

    1. Patients' records in a tertiary care university hospital were reviewed. Fifty-four Black patients presenting with phaeochromocytoma between 1980 and 2009 were included. Two patients had VHL syndrome (submitted for publication). This diagnosis of VHL syndrome was made in the pregnant woman (Case 9) who had bilateral adrenal phaeochromocytomas. There were no clinical features of VHL syndrome; however, MRI showed a haemangioblastoma in the cervical spine. Genetic testing revealed a missense mutation (c.499C>T) in the VHL gene, located on chromosome 3p26-p25. Her 6-year-old daughter screened positive for the mutation; her 8-year-old son was negative. Both mother and daughter will be investigated further and monitored long-term. ACMG PP4

    1. 5 nonsense mutations

Among patients prospectively followed by the members of the French VHL Study Group, 35 consecutive patients with well-documented endocrine pancreatic tumour (20 women and 15 men; median age,37 [21Y-57] years) were included. The data from 7 patients with EPT have been partially reported previously in Hammel et al., 1995;2000. All 35 patients had VHL germline mutations and were assessed for the presence of pheochromocytoma and renal carcinoma in addition to pancreatic tumours. The presence of other phenotypes was not described. Patient no.30 from family 64 had a nonsense mutation (c.217C>T) and presented with pancreatic endocrine tumour and renal carcinoma. The patients' phenotypes are highly specific to VHL disease (PP4) and nonsense mutation supports PVS1.
