    2. Germline mutations were identified in 300/469 (63%) of the families tested; 137 distinct intragenic germline mutations were detected.

Germline mutational data on VHL was gathered from various countries to assess variation in germline mutations that produce disease within different populations. Families were grouped according to three possible outcomes of disease: renal cell carcinoma without pheochromocytoma, renal cell carcinoma with pheochromocytoma, or pheochromocytoma alone. SSCP and denaturing gradient gel electrophoresis identified mutations in 300/469 (63%) of families affected with VHL. This germline familial VHL mutation (N78S (c.233A>G)) was taken from multiple databases including: the Department of Uroglogy, Yokohama database, the Munich, university of Freiburg and Institute of Pathology database, the National Cancer Institute database, and the Cambridge, Department of Pathology database, with an unknown number of members belonging to Family IDs 9YO, 51CE, 4327NCI, 160CA. It was reported in the paper as c.446A>G due to a difference in transcript reference. As reported in Table 3, this mutation is found in VHL families without pheochromocytoma, and with renal cell carcinoma.

    1. Genotype-phenotype correlations of 573 VHL patients were analyzed and confirmed that higher risk of pheochromocytoma is associated with missense mutations that result in substitution of a surface amino acid or that disrupt the structural integrity of the VHL gene product (pVHL). Age-related risks of retinal hemangoblastomas and renal cell carcinoma were higher in patients with nonsense or frameshift mutations. This nonsense mutation (c.264G>A) was found in a VHL family with 3 affected members, 1 had retinal hemangioblastomas, 1 had cerebellar hemangioblastomas, 2 had renal cell carcinoma, and 1 had pheochromocytoma. There is very strong evidence for pathogenicity in the form of a nonsense variant in a gene where loss-of-function is known mechanism of disease (ACMG code: PVS1).

    2. Genotype-phenotype correlations of 573 VHL patients from 200 kindreds were analyzed. Higher risk of pheochromocytoma was associated with missense mutations that result in substitution of a surface amino acid (p<=0.004) than for patients with missense mutations that disrupt the structural integrity of the VHL gene product (pVHL) or other loss of function mutations (either germline deletions or truncating mutations). This missense mutation (c.533T>C), affecting residue on the surface of pVHL, was found in a VHL family of 3. All family members had retinal hemangioblastomas and cerebellar hemangioblastomas. Two had renal cell carcinoma. Another family in this study was observed with a L178R (c.533T>G) mutation. ACMG codes as follows: Supporting evidence of pathogenicity because Missense variant in a gene that has a low rate of benign missense variation and where missense variants are a common mechanism of disease (PP2).

    3. This missense mutation (c.332G>A), affecting surface residue of pVHL, was found in 3 VHL type 1 families with 10 affected individuals altogether. Four of 10 individuals had retinal angioma, 6 had cerebellar hemangioblastoma, and 2 had renal cell carcinoma.

    1. c.565delG

Data was collected from 82 VHL mutation carriers in the Dutch VHL surveillance program. One patient was found with this frameshift mutation in the VHL gene. The patient was 30Y at the last follow-up and presented with: cerebellar hemangioblastoma and renal cysts. Family history was not described. This cohort was published later in PMID 31087189.

    1. The data of 18 patients with solid renal tumours associated with VHL disease were analyzed. Seventeen patients had a VHL family history, the only patient that had no family history of VHL was patient 12. The patients came from 12 Chinese families and 5 were from the same family. Extrarenal manifestations of VHL disease were detected in 17 patients. Of 18 patients, 15 (including 6 asymptomatic) volunteered for DNA testing as previously described. Patients 17 and 18 had genetic testing and were found to have the same germline mutation c.332G>A (Ser111Asn). This missense mutation is noted as c.545G>A in the paper due to a difference in the VHL reference sequence notation used. It is unclear if these patients are related. Both patients 17 and 18 had family study DNA test as their reason for treatment. Patient 17 was a 39-year-old female with a medical history of solid renal tumour, multiple renal cysts, CNS hemangioblastoma, pancreatic cyst, retinal angioma. Patient 18 was a 45-year-old male with a medical history of synchronous bilateral solid renal tumors and CNS hemangioblastoma.

    1. The VHL germline mutation previously detected in the white blood cells of this patient was C407T, Ser65Leu. Mutation analysis of all the VHL exons revealed this germline mutation in exon 1 in all tumors analyzed; however, no additional somatic mutations were detected (Table 1)

A 44-year-old man presented with a generalized epileptic attack. Diagnostic workup revealed a cystic left cerebellar tumor, which on histologic examination proved to be a hemangioblastoma. Identification of a VHL germline mutation (c.194C>T, reported as c.407C>T due to a difference in transcript sequence) in a blood sample of the patient confirmed the diagnosis. The patient also had retinal hemangioblastomas, and clear cell renal cell carcinoma around 51Y. Family history was positive for VHL disease, although no further information provided. The patient died 10 years later at 54Y from metastases to the heart and autopsy also confirmed pheochromocytoma and pancreatic endocrine tumour. Somatic and chromosomal imbalances in the tumours of this patient were also documented. ACMG PP4 phenotypes and family history consistent with VHL disease.

    1. Seven VHL mutations were identified in 10 unrelated, Swedish families. Direct genome sequencing through Southern blot identified this missense mutation (c.223delA) in a family (#3) of 8 individuals. One affected family member had renal cell carcinoma, 2 had retinal angiomas, and 2 had hemangioblastomas of the central nervous system. Other information on the affected member and relatives was not available.

    2. A retrospective cohort study included all the VHL patients diagnosed at Peking University First Hospital (Beijing, China) prior to June 1 2016. A total of 291 patients from 115 different families were included for analysis. A male patient (family #16, patient #45) was confirmed with this missense mutation (p.R82P(c.245G>C)) in the VHL gene. He was diagnosed with CNS hemangioblastoma at 40Y and multiple pancreatic cysts or tumours at 26Y. This patient was also published in a paper by Wang et al., 2018 (PMID: 29330336).

    1. 665 T/CI151T

42 CNS hemangioblastoma samples were obtained from 31 patients, who were consecutively treated between 1993 and 1997 at the Department of Neurosurgery of the Freiburg University Medical Centre were analyzed for VHL mutations. One patient of 17 presenting with VHL germline mutations presented with the I151T (c.452T>C) mutation (stated as c.665T>C in the paper which used the Latif sequence as reference). Single strand conformation polymorphism and southern blotting for variant identification in the DNA found in blood samples. The patient was established as having VHL disease by standard of Neumann at al., 1987. The patient presented with a single vermis tumor.

VariantEvidence: Variant is declared as “c.499C>T (Q96X)” by the authors, this was treated as a legacy variant and mapped to c.286C>T, Q96*. This study found coexpression of Epo and EpoR in 11 RCCs and in 10 benign renal cysts in a spectrum of VHL renal disease suggesting that Epo/EpoR coexpression is present in both benign and malignant stages of renal neoplasia. Six benign cysts stained for Epo but not EpoR. The researchers detected loss of heterozygosity of a wild-type VHL allele in different RCCs with microsatellite markers, D3S1110 and D3S2452, flanking the VHL gene

    1. NM_000551.3:c.95A>G(p.Glu32Gly)

This abstract presented the case of a 37-year-old caucasian male with a right adrenal pheochromocytoma. Genetic testing revealed NM_000551.3:c.95A>G(p.Glu32Gly) mutation in the VHL gene. The patient also had significant family history in grandmother who had an adrenalctomy and his son diagnosed with hypertension. Genetic testing for family member was not reported in the abstract.

    1. One hundred and sixteen unrelated VHL disease patients have been investigated by molecular genetic analysis. Twenty-two of these were previously shown to have germline deletions on Southern analysis, and in the present study the remaining 94 patients were screened for intragenic mutations via SSCP and heteroduplex analysis of 855 bp of the VHL coding sequence and 800 bp of the 3' UTR. An intragenic mutation was identified in 55 (59%) out of 94 patients without germline deletions on Southern analysis. In all patients with multiple affected family members the missense mutation segregated with the disease. Large deletions or intragenic mutations predicted to cause a truncated protein were found in 36 of 53 families without pheochromocytoma (VHL type 1) and 2 of 12 families with pheochromocytoma (VHL type 2). This study also investigated the frequency of different mutation classes between kindreds with and without pheochromocytoma. The p.E94 (c.280G>T) missense mutation (noted as p.E165 (c.493G>T) in the paper due to a difference in reference transcript used) was found in at least one patient in kindred 51. Pheochromocytoma was not observed in any members of this kindred (implying the affected patient(s) had VHL disease type 1). No other familial phenotypic information could be inferred from the paper.

    1. In family 4, the proband was a female who had a bilateral pheochromocytoma at the age of 46 years that recurred locally with liver metastases a few years later. Her brother had a monolateral benign pheochromocytoma. The proband also had a melanoma and bilateral renal cysts. A VHL sequence variant IVS2 +43 A>G was found in this case and also in one other unrelated sporadic pheochromocytoma. Their VHL mRNA integrity is currently being assessed. The proband had no cerebellar or spinal NMR finding or retinal alterations. Molecular analysis showed the following sequence variants: VHL IVS2 +43 A>G, RET c.2071 G>A p.G691S, SDHD c.34 G>A p.G12S, IVS2-29 A>G, c.204 C>T p.S68S, and IVS3-32 T>C.

    2. The authors described the growth kinetics of 96 renal tumours in 64 VHL patients (32 males, 32 females). All patients had VHL disease with renal involvement. 54/64 had undergone renal surgery or a radiofrequency thermal ablation procedure for RCC. In 10/64 patients, at least one or more RCC was diagnosed. However, at the time of diagnosis, an indication for treatment was not yet present. For all patients, several CT/MRI scans existed for retrospective description of tumour growth kinetics. 10-ml EDTA anticoagulated blood samples were available from each patient for mutation screening of all exons of the VHL gene. Genomic DNA from blood samples was extracted using standard methods. Small deletions/insertions or point mutations were identified by DNA sequencing followed by complete mutation screening using PCR/single-strand conformation polymorphism and denaturing high-performance liquid chromatography. A germline missense variant p.W88R(c.262T>A) was identified in two patients: patient #43 and patient #46. This mutation was noted as c.475T>A in the paper due to a difference in reference transcript used (see Table S1). No family information is available for either patient, and there is no indication these patients are related.

    1. 41 non-related patients with retinal capillary hemangioma presenting in the Benjamin Franklin University Eye Clinic between 1988 to 1999 Germany were evaluated for the presence of other VHL lesions. VHL disease was diagnosed in 33 patients based on the presence of other severe VHL lesions (spinal hemangioblastoma, cerebellar hemangioblastoma, renal cysts, pancreatic cyst, renal carcinoma, and pheochromocytoma), molecular genetic testing, and/or family history. VHL germline mutations were identified in 27 out of 29 VHL families screened with molecular genetics. Blood samples were genetically tested using PCR-SSCP sequencing. One patient with VHL-related retinal angiomatosis was found to have germline nonsense (c.280G>T) mutation in the VHL gene. 1 other affected relative was also evaluated and found to have this mutation but no phenotype information was provided. This variant was reported as c.493G>T (AF010238.1) and mapped to p.E94* (c.280G>T) (NM_000551.3). It's unclear which other VHL-related lesions were identified in these patients.

    1. A 3 year-old girl was admitted in the Department of Pediatric Oncology following several months history of febrile episodes, sweating and irritability. Symptoms were first interpreted as being of infectious causes, and that is why she received repeated antibiotic therapies. There was no improvement, and that is why more investigations were performed. An abdominal ultrasound performed in a pediatric hospital revealed a right adrenal tumor mass that prompted the transfer to our unit.Family history: the patient’s mother aged 38 was diagnosed with bilateral pheochromocytoma resected when she was the age of 18 years. She presented several relapses, the most recent one at the age of 32, when she was also diagnosed with cerebral angioma. Genetic testing revealed the presence of the VHL c 244 C>G (p. Arg 82 Gly) heterozygote mutation, while the tests for RET, SDHB, SDHC and SDHD were negative. The clinical examination of our patient revealed: pallor, paroxysmal febrile episodes with irritability and sweating, tachycardia (130/min), hypertension (170/126 mmHg). There was no palpable abdominal mass.Abdominal ultrasound: well delimited tumor of the right adrenal gland, 37/34/35 mm in diameter, irregular with transonic inclusions.Magnetic resonance imaging (MRI): right adrenal tumor, 35/42/43 mm in diameter, well delimited, irregular, with marked heterogenous contrast intake, presenting cystic and necrotic areas (Figure 1).Open in a separate windowFigure 1Right adrenal tumor.Serum markers: urinary vanillylmandelic acid (VMA) 31.1 mg/24 h (normal <6.6), neuron specific enolase (NSE) 59.79 ng/ml (normal <17), ferritin 306.6 ng/ml (normal range 6–67), lactic dehydrogenase (LDH) 333 IU/L (normal range 125–300).Genetic examination: VHL c 244 C>G (p. Case: M, 15Y0M, Serbian

      DiseaseAssertion: Asymptomatic

FamilyInfo: Families were selected based on their clinical data and family history. Patient and his brother (CV2) are both currently asymptomatic, but possess the same variant as their father (CIV2), who was diagnosed with retinal hemangioblastoma at 12Y0M. Then, multiple pancreatic and kidney cysts were detected at 29Y0M and left kidney cancer was found at age 35Y0M. Patient’s grandfather, great grandfather and great uncle (CIII1; CII1, CII2) had retinal capillary hemangioma, CNS hemangioblastoma and renal cell carcinoma. Out of the three of them, only the great grandfather had genetic testing performed and was found to have the same mutation as the patient. See pedigree in figure 1.

    2. c.463G>A/p.Val84Met

The VHL gene was analyzed in 3 families from a Serbain population. Genomic DNA was screened by SSCP analysis and patients with variant bands were subjected to sequencing and confirmatory re-sequencing. 3 male members (BI1; BII2; BIII3) from Family B had pheochromocytoma. Two of them (BII2; BIII3) were genetically tested and confirmed to possess the missense V84M (c.250G>A) VHL mutation, reported in the paper as V84M (c.463G>A) due to a difference in transcript sequence. Patient BII2 and BIII3 were diagnosed at 40Y and 1Y with pheochromocytoma, respectively.

540 patients from 187 unrelated Chinese VHL families at the Peking University First Hospital were all screened for VHL clinical features and germline mutations. Of these, clinical characteristics were observed in 258 patients from 80 unrelated Chinese families. These were reported in this study, grouped by the 21 mutations found. PCR and MLPA on peripheral blood detected the VHL mutations. Possessing the p.S65L (c.194C>T) mutation, 24 members total from an unknown number of families were described, and phenotypic information was given for this group. With ages ranging from 15-65Y, hemangioblastomas were observed in 15/24 members, retinal capillary hemangioma in 2, renal cell carcinoma in 9, pancreatic cysts in 11, pheochromocytoma in 1, and a genital system tumor (epididymis or broad ligament) in 4 members. This cohort has been previously published in PMID 29330336; 28388566.

FamilyInfo: The patient’s mother and two brothers had died with cerebellar hemangioblastoma, although genetic testing was not performed. One of the patient’s four children, a 46Y male, also presented with a small renal tumour, renal cysts, and pancreatic cysts; he was found to carry the same mutation the patient carried. One of the patient’s grandsons, a 10Y male, was also found to carry the same mutation as the patient but was asymptomatic. Genealogical inquiry also revealed that the patient was distantly related to a known VHL family; one member, a 47Y male, was previously operated on for CNS hemangioblastoma, RCC and symptomatic pheochromocytoma and was found to carry the same mutation as the proband. Another female member had previously presented with retinal hemangioblastoma before becoming deceased at 37Y. The paper also asserted that the patient’s grandmother and two distant relatives carried the mutation, although genotyping was not performed and phenotypic information was not provided.

CaseHPOFreeText: The patient was originally diagnosed with metastatic bilateral clear cell renal cell carcinoma at 68Y and underwent chemotherapy for three months. The metastases were later reinterpreted to be a cystic hemangioblastoma and a benign serous cystadenoma. The patient was also found to have a non-secreting adrenal pheochromocytoma at 73Y and a clear cell renal cell carcinoma metastasis in the thyroid at 74Y. The patient had also previously presented with retinal lesions, although detailed records were unavailable.

GenotypingMethod: Genetic analysis was performed at the laboratory of hematology of the Radboud University Medical Center in Nijmegen. Ion semiconductor sequencing (Ion-Torrent) was used to analyze the VWF gene from peripheral blood obtained in EDTA tubes or from saliva samples. The 52 exons of VWF gene were analyzed including ±20 bp exon-intron boundaries. If no VWF gene variant was detected, multiplex ligation-dependent probe amplification (MLPA) analysis was performed to find large deletions or duplications. Thus, MLPA was performed in 183 patients (46.9%). All variants were confirmed with Sanger sequencing.

    1. family A, a single nucleotide mutation, T>C, in VHL exon 1 was detected in proband A01 and offspring A002 (without disease), and both were T homozygotes;

Three families with VHL syndrome were enrolled in this study in 2012 to identify variants. Whole exome sequencing (WES) was performed on members (3 generations) with VHL syndrome of all three families. The three families included 9 patients with confirmed VHL syndrome, but according to family history and clinical diagnosis, some members of all 3 families had VHL syndrome. 9 patients were found to have CNS Hemangioblastomas (Hbs), but not all of them had VHL gene mutations. This means CNS Hb in VHL syndrome is not only related to VHL gene mutations. Therefore, mutations in CNS HB-related genes were examined and a total of 34 mutations in the following 21 genes were found: BCLAF1, CACNA1B, CROCC, FRG1, KCNJ12, MAP2K3, MUC4, MUC5B, MUC6, OR4C3, OR4 M2, OR9G1, POTEC, PRIM2, PRSS1, SLC25A5, SLC9B1, SUSD2, TAS2R19, TAS2R31, and TUBA3E. In family 1 (A), the WES results showed a homozygous P86S(c.256C>T) missense variant in exon 1 of the VHL gene in proband A01 and offspring A002. Proband A01 had CNS Hb, clear cell renal cell carcinoma, and pancreatic cystic disease, while the daughter has no disease. Proband's mother, two uncles, and aunt-in-law had hemangioblastomas and are no longer alive. Proband's cousin also had a hemangioblastoma.

A cohort of 36 PCCs and four functional PGL tumours were analysed by exome-seq and high-density SNP-array analysis using tumor paired blood. This specified germline VHL mutation (tumor ID: V-PH-29T) belonged to a female diagnosed with bilateral phaeochromocytoma at age 8Y, surgery at 10Y. She also had a personal history including extra adrenal abdominal phaeochromocytoma and lymph node involvement at age 8yrs. The p.V84L (c.250G>C) VHL mutation was also accompanied by an additional germline variant: MEN1: c.512G>C (p.Arg171Pro). It was stated that there were no familial cancers/syndromic illnesses with this patient. Additionally, this patient was likely previously published in PMID: 26796762 (ID: WM02).

37 Danish patients diagnosed with VHL before 18 years from a prospective cohort study from Denmark's National vHL Research Database (Binderup et al., 2013) were assessed. One family with 3 patients (Family ID 5) was found with a germline truncating mutation in the VHL gene (Q145*; c.433C>T). Patient 1 was diagnosed at 0Y and had 1 retinal hemangioblastoma at 17Y. Patient 2 was diagnosed at 3Y and had 13 CNS hemangioblastomas at 11Y, 13Y, 15Y, and 17Y, 1 retinal hemangioblastoma at 13Y, and 2 pancreatic cysts at 16Y. Patient 3 was diagnosed at 9Y and had 1 retinal hemangioblastoma at 12Y, 1 pancreatic cyst at 12Y, 1 CNS hemangioblastoma at 15Y, and kidney disease at 12Y. They were 28Y, 20Y and 22Y respectively at the end of the study.

    1. missense germline mutation in exon 3 of the VHL gene (p.Leu158Pro)

A case series described 3 patients with endolymphatic sac tumor and VHL disease. Case #2 showed a female patient who was previously diagnosed with VHL at 30Y and previously had cerebellar hemangioblastomas, (unspecified) renal cancer, retinal hemangioblastoma. At 46Y, the patient was found with endolymphatic sac tumour. Genetic testing confirmed a VHL germline mutation in the patient (c.473T>C) but not in the patient's family, suggesting a de novo mutation.

A retrospective series of case reports were analyzed to detect and compare changes in VHL disease-associated tumors before and after sunitinib treatment. Sanger sequencing of blood identified the mutations discussed. Two members of a family tested positive for a c.239G>T VHL mutation leading to a p.Ser80Ile protein change. The first family member (patient 1) described is a 44 year old female, whose presentations included bilateral adrenal pheochromocytoma at 29, pancreatic neuroendocrine tumor at 38, renal cysts and renal cell carcinoma at 42, and cerebellar hemangioblastoma at 43. Her daughter underwent cortex-sparing adrenalectomies for bilateral pheochromocytomas as well as hemangioblastoma resection in her left cerebellum at age of 19 years, although testing for VHL is not stated. Patient 2, the brother of the first patient, presented with bilateral adrenal pheochromocytoma at 38, cerebellar hemangioblastoma at 46, and clear cell renal cell carcinoma at 54.

Between 1994 and 2001, the Dutch VHL working group conducted DNA analysis on 146 probands with VHL-associated manifestations. DNA of probands was extracted from peripheral blood samples. Exons 1, 2 and 3 of the VHL gene and their flanking untranslated and intronic coding sequences were amplified by PCR and subjected to semi-automated sequence analysis. Screening for structural rearrangements, including deletions, involving the VHL gene was performed by Southern blot analysis and the multiplex ligation-dependent probe amplification (MLPA) assay. Deletions encompassing the entire VHL gene were confirmed by FISH analysis on lymphocytes from affected individuals. VHL mutations were identified in 43 families. This mutation (c.509T>A) was detected in 14 VHL patients (family no. 18). Six had retinal hemangioblastoma, 9 had CNS hemangioblastoma, 7 had renal cell carcinoma, 8 had renal cysts, 4 had pheochromocytoma, 5 had pancreatic cysts, 2 had endolymphatic sac tumors, 1 with adnexal papillary tumor of probably mesonephric origin, and 5 had hepatic cysts.

A study investigated paraganglioma/pheochromocytoma tumors in a retrospective cohort and a prospective cohort. Some of these tumors were found with germline VHL mutations. Tumour ID 92 (from a patient who was 29 years old) was paraganglioma and a germline c.430G>C mutation in the VHL gene. Other phenotypic and demographic information was not available.

    1. VHL Variants

Description This report studied the prevalence of germline variants in Japanese PPGL patients. Of the 370 PPGL original Japanese probands, this cohort consisted of 15 patients with pheochromocytoma and/or paragangliomas with germline VHL varaints. Blood samples were used to examine 7 genes: MAX, SDHB, SDHC, SDHD, TMEM127, VHL, and RET, using Sanger sequencing. Of these 15 VHL patients: 10 had bilateral PCC, 4 had PCC with abdominal/thoracic PGL, and one had metastatic PPGL. The variant R161Q (c.482G>A) was one of the 15 found in a patient with PPGL. Their family history or further manifestations were not detailed.

BCVA was 20/25, and a peripheral RCH with a macular pseudohole was observed at the fundoscopic examination (Figure 3). The tumor was treated using laser photocoagulation and required several treatment sessions. One week after the first laser session, the pseudohole progressed to a lamellar macular hole (LMH) and after some months, to a FTMH. BCVA worsened to 20/63 in 2012, and pars plana vitrectomy was successfully performed: The macular epiretinal membrane and internal limiting membrane were peeled. During follow-up, a retinal tear was detected (in 2014), and it was treated using a barrage laser. The last follow-up visit (in September 2019) revealed a good macular profile and RCH regression; BCVA in OS was 20/25. During the follow-up, OS did not develop lesions.

Among patients prospectively followed by the members of the French VHL Study Group, 35 consecutive patients with well-documented endocrine pancreatic tumours (20 women and 15 men; median age 37 [21-57 years],) were included in this study. All 35 patients had VHL germline mutations and were assessed for the presence of pheochromocytoma and renal carcinoma in addition to pancreatic tumours. VHL mutation analysis was performed on DNA isolated from peripheral blood lymphocytes. PCR analysis and direct sequencing of the 3 exons of VHL was performed for all the DNA. If no mutation was detected, Southern blot analysis was performed to detect large rearrangements. Patient no. 14 from family 12 had a missense mutation in the VHL gene (c.245G>C) and presented with pancreatic endocrine tumour, pheochromocytoma, and renal carcinoma. Suggested ACMG evidence codes: The patient's phenotypes are highly specific to VHL disease (PP4).

    1. p.(Asn78Ser)

VHL germline mutation carriers who had an ophthalmologic examination at the Erasmus Medical Centre or the Rotterdam Eye Hospital between 1984 and 2017, and were diagnosed with retinal hemangioblastoma (RH) were included in this study. A diagnosis of RH was based on the clinical appearance of a vasoproliferative retinal tumour. Peripheral blood samples from every member that had clinically suspected VHL disease of each family were collected for VHL mutation analysis. A total of 21 patients from 10 families were included on the basis of both a VHL germline mutation and ocular manifestations. Copy number analyses were carried out by multiplex ligation dependent probe amplification (MLPA) and fluorescence in situ hybridization (FISH). Mutations were described as one of the following two groups: truncating (complete deletion, partial deletion, nonsense mutation, splice site mutation, duplication and insertion) or missense. Ocular manifestations were not reported on an individual level, but rather were broken down based on the two groups. From Pedigree (family) 4, one individual was suspected of VHL disease and mutational analysis revealed a germline N78S(c.233A>G) missense mutation. This individual had RH development.

    1. Case report: We present a case of a compound heterozygote patient with von Hippel-Lindau disease and familial erythrocytosis type 2. One of the mutations found in our patient, c.416C>G (p.Ser139Cys) of the VHL gene, has not been previously reported. This case is the second one reported where von Hippel-Lindau disease and familial erythrocytosis type 2 coexist in the same individual.

      KindredID: 1

      Case: M, 9Y5M, Ethnicity Unknown

      DiseaseAssertion: VHL

FamilyInfo: This patient has a large family history of VHL disease (4 generations). 12 other members are reported in Fig 1. as having VHLd although their genotyping is not availably discussed. Only the proband’s mother was said to have the disease and the Exon 3 deletion, passing away due to complications of a hemangioblastoma in the CNS.

      CasePresentingHPOs: HP:0030424 (Epididymal cyst)

CaseHPOFreeText: This is a pediatric patient with both familial erythrocytosis type 2 and VHL disease. First diagnosed with polyglobulia (polycythemia) at 8Y. A physical examination showed a tumor in the right testicle (cyst in the right epididymis)

    1. start codon variant (c.3G>A, p.Met1Ile) and a missense variant (c.631A>C, p.Met211Leu)

Out of 48 Danish families presenting with renal cell carcinoma, one patient was found to harbor the germline mutations c.3G>A (p.Met1Ile) and c.631A>C (p.Met211Leu) in the VHL gene. This patient was diagnosed with clear cell renal cell carcinoma (age of onset: 28). He had a family history of testicular cancer in his maternal uncle. The patient has been through the national screening program for putative VHL, which includes MRI of the cerebrum, spinal medulla and abdomen, neurological examination, auditory tests, ocular examination and blood work for chromogranin A and metanephrines, with normal results. The proband also presented with the germline variant c.1729+8T>C in BAP1. Unclear if/which other family members were genetically tested.

Patients with ELSTs were identified and included in the International Endolymphatic Sac Tumor Registry. The registry contained 93 patients as of July 2014, and of those, 25 were sporadic, and 68 associated with VHL. Patients with ELSTs and clinically confirmed diagnoses of VHL were included in this study. Statistics of this VHL subgroup are as follows: mean age 30, 62% female, 91% symptomatic, 54% with retinal angioma, 72% with CNS hemangioblastoma, 46% with renal cell carcinoma, 19% with pheochromocytoma, and 12% with pancreatic neuroendocrine tumour. In the data from this study, only endolymphatic sac tumours can be confirmed as a phenotype for this mutation (c.533T>C), which was found in 1 individual from France.

    1. Supplemental Table S1

Six patients with classic VHL disease (VHL patients) and 6 healthy control participants took part in the study. The patients were free from any associated neoplasia at the time of the study and had no other medical disorders. With respect to the VHL patients, the aim of the authors was to determine whether any aspects of the CP phenotype are detectable in classic VHL disease. One 53Y female VHL patient and a 27Y male VHL patient with unknown phenotypes were described to have the Ser111Asn VHL mutation.
