    1. Because of their greater efficiency in rescuing exon 2 splicing, we decided to test the effect of AMOs 1, 2, and 3 on cultured fibroblasts obtained from a patient carrying the c.-32-13T > G mutation in heterozygosis with an unknown mutation that abrogated the expression of the other allele.

Patient1: the efficacy of AMOs 1, 2, and 3 were tested in cultured fibroblasts from this patient

Under basal conditions, the fibroblasts retained 20% enzymatic activity compared to WT cells; treatment with combination of the AMOs showed rescue of normally spliced GAA mRNA, this was confirmed with real-time qPCR

    1. There is a description of 22 Dutch families carrying the c.-32-13T>G mutation, where the course and severity of the disease varied substantially among most of the siblings despite having identical genotypes. This suggests that other components, such as lifestyle, nutrition, environmental influences and other genetic or epigenetic factors, may contribute to explain such clinical inter and intra-familiar variability

HPOsFreeText Patient had severe intellectual disability. Her spine was normal, but long bones were osteoporotic. Upon exam at 5yo, she had mild kyphosis of upper thoracic spine, mild sternal depression, marked genu valgum, and arachnodactyly. Patient was also myopic with bilateral lens dislocation. She had prominent recurrent malar flush, long and thin face, high-arched palate, malocclusion and mitral valve prolapse.

(PMID: 20329304): Patient was born at term after normal pregnancy weighing 3.4kg. Normal neonatal course, but with delayed milestones. Speech at 5yo was monosyllabic , but verbal comprehension was good. No convulsions at this time. She had a palpable liver, 2cm below margin, but had normal consistence. She had an abnormal EEG with non-specific diffuse changes and excess slow-wave activity. Patient's IQ was 36.

(PMID: 20329304): "Full hemogram, routine urine analysis, liver function tests, serum ion and iron-binding capacity, plasma magnesium, serum calcium, inorganic phosphate, fasting blood sugars, blood urea nitrogen, and bone marrow examination were all normal". Heavy homocysteine spot shown on chromatogram of urinary amino acids.

      CaseMethod1 PCR

      CaseMethod2 Sanger

CaseGenotypingMethod Created a bacteria expression system in E.coli by substituting one of three fragments of (PCR amplified)patient cDNA into WT CBS sequence already subcloned in expression plasmids. Bacteria were screened based on which ones did not have CBS activity. Those that did not have activity were thought to have LOF variants in CBS. Plasmids containing the inactivating fragments, ones cut with NcoI/XmaI, were sequenced.

GeneFunction: GO:0004353 (glutamate dehydrogenase [NAD(P)+] activity)

GeneAlteration: Recombinant adenoviruses encoding WT human GLUD1 and the S498L variant were generated via an expression system, with the human GLUD1 gene being amplified and sub-cloned into a plasmid with a promoter. Digestion using EcoRV was used to discriminate between the wild-type and the mutant with sequencing verifying each step of the cloning. This adenovirus was amplified in HEK-293 cells and INS-1E β-cells were transduced with it. Mouse and human hepatocytes were isolated and seeded before being transduced with the same adenoviruses. The transfection efficency was confirmed via immunoblotting.

FunctionalEvidence: The enzymatic activity was evaluated in both wild-type and mutant mice, with both showing very low activity without glutamate concentrations and the mutant showing higher activity when glutamate was increased. The reductive direction using α-ketoglutarate as substrate was also tested with similar increased results for the mutant. Sensitivity of both lines to the reaction activator (ADP) and inhibitor (GTP) levels were tested, showing that the mutant was more sensitive to the ADP activator and slightly reduced for the GTP inhibitor (particularly in the presence of ADP). Mitochondiral function in wid-type and mutant cells was assessed, demonstrating that glucose response from mutant and WT cells was preserved but that glutamine stimulation abnormally generates ATP in mutant cells.Calcium stimulation was also activated by both glutamine and glucose in mutant cells while only being affected by glucose in WT. Studies in mutant and WT-transfected mouse and human islets showed increased insulin and ammonia production in mutants from stimulation through this responsive glutamine sensitivity. All of this supports GLUD1's pathogenic mechanism, specifically causing a hyperctive mutant protein that responds to glutamine levels rather than simply glucose.

When the soluble fraction was examined for PCBD1 activity, no activity was observed. The authors note that the absence of activity could be due to the absence of the protein in the soluble fraction alone, instead of being indicative of the mutation causing a loss of enzyme activity. Only if the mutant recombinant protein could be retained in the soluble fraction can the true enzyme activity be assessed.

AddCaseInfo: Family E in the article, and the proband had an older sibling that also carried the homozygous mutation in PCBD1, she was not noted as having increased Phe plasma levels, nor was ever treated with BH4 or low-Phe diet.

VariantEvidence: The mutant Q97X protein was expressed in E.Coli to determine the solubility of the mutant enzyme and the activity. The mQ97X fragment was mostly found in the insoluble fraction of the supernatant. The remaining fraction obtained in the soluble fraction did retain some activity in vitro. Interestingly, when more Q97X was obtained in the soluble fraction (by growing the E.coli at 20 degrees C), the Km (Michaelis constant) was 2-3 times higher than wildtype (text only, no figures to compare).

FamilyGenotypingMethod: ALDH18A1 was directly sequenced upon investigation for a causal role in SPG9, due to being within the linked region 10q23.3-q24.2, not having been discarded as a candidate gene, and mutations having been known to cause cutis laxa. Previous haplotype analysis also confirmed the linkage to this specific genomic region. This revealed one mutation in each pedigree that strictly co-segregated with the disease. Exome NGS sequencing within the interval showed only two variants were unknown in ExAC with the MYOF substitution unlikely to be disease causing due to poor conservation.

The mutant E86X/E87X fragment was mostly found in the insoluble fraction of the supernatant. The remaining fraction obtained in the soluble fraction did retain some enzyme activity in vitro. Interestingly, when more E86X/E87X was obtained in the soluble fraction (by growing the E.coli at 20 degrees C), the Km (Michaelis constant) was similar to wildtype levels (text only, no figures to compare).

For the C82R/C81R variant was analyzed by Ayling et al., 2000 (PMID:10924272) and was found to still be retained in the soluble fraction in bacteria expressing the recombinant protein. Furthermore, enzyme assays show that the soluble C82RPCBD1 variant retained activity, albeit at 40% of the wildtype.

Targeted capture was performed and reads were aligned to the human genome. This identified five putatively linked loci with multipoint LOD scores (+1.64 to +1.95) and WES found two missense variants that fits the above criteria and segregates with the disease in ALDH18A1 and PDZD7. Found variants were called and annotated. Segregation in all individuals was checked through Sanger sequencing.

These were tested with 28 x-chromosomal probes for heterozygosity in carriers, leading to an identified area near Xq28. The 30 genes in this region were identified and five were noted to have high heart and skeletal muscle expression. All five were sequenced and ORFs created. After PCR amplification and direct sequencing of the TAZ gene (G4.5.), each family was seen to have a unique variant that segregated with the disease.

    1. RESULTS

      NonHumanModel: Vervet Monkeys (Chlorocebus pygerythrus)

GeneAlteration: PCR amplification and direct sequencing indicated that all of the vervet monkeys observed and one of the malbrouck monkeys had the TTR Ile122 allele, as opposed to the the other primates (including humans) that harbor the Met122 allele. Other than this variant, the sequence was identical to three other species of monkeys that did not demonstrate amyloidosis phenotypes including 95-98%, 93-94%, and 76-81% amino acid homologies to humans.

ModelPhenotype: HP:0031327, HP:0003216, HP:0011675, HP:0012664 (Transthyretin cardiac amyloidosis, Generalized amyloid deposition, Arrhythmia, Reduced ejection fraction)

HumanPhenotype: HP:0001627, HP:0011034 (Abnormal heart morphology, Amyloidosis)

      HumanFreeText: N/A

ModelEvidence: Aged vervet monkeys were investigated for a naturally-occuring spontaneous systemic amyloidosis and accompanying phenotypes. PCR amplification and direct sequencing of these monkeys and others showed that the vervet monkeys alone harbored the Ile122 allele, as opposed to the more common Val122 allele in other primates and humans. Histopathological investigations showed cardiac amyloidosis and rigid amyloid fibers in the heart, identical to that observed in aged humans with the allele in question. These deposits were also found in other systems such as the gastrointestinal tract, adipose tissue, lung, tongue, and vascular walls, also mirroring the multiple locations they were found in humans. Immunoblotting confirmed that amyloid proteins extracted from the cardiac specimens reacted with anti-TTR antibody. Other cardiac symptoms were observed that mirror those in humans, such as arrythmia and reduced ehection fraction. Real-time PCR showed that these monkeys and humans shared the majority of TTR expression patterns. Recombinant monkey TTRs showed a greater tendency to dissociate to monomers under the influence of urea-mediated denaturation stress than recombinant TTR of the cynomolgus monkey, which had the Val122 allele. ThT-binding assay also showed a higher number of amyloid fibers in the recombinant than the cynomolgus.

    1. RESULTS

      NonHumanModel: Mouse (Mus musculus)

GeneAlteration: CRISPR-Cas9 gene editing was used to produce gene-edited mouse zygotes. This was accomplished by designing a guide RNA (gRNA) with the CRISPR design tool and microinjecting the gRNA complex along with Cas9 mRNA and a ultramer repair template including the single deletion into mouse zygotes. This directs double-strand breaks in the genome with homology directed repair causing the precise genome edit necessary. These were injected into psuedo-pregnant C57BL/6J mice. After birth, tail DNA was genotyped via PCR and Sanger seuqencing to confirm the mutation. One heterozygous 525delT mouse was crossed and the offspring backcrossed onto DBA/2J background until the F5 generation confirmed homozygous for the severe LTBP4 allele which results in a more severe disease progression. RT-PCR confirmed the presence of the deletion and loss of γ-sarcoglycan was confirmed via immunoblot analysis and immunofluorescence microscopy.

      HumanFreeText: N/A

ModelEvidence: This model has many positive aspects compared to the default mouse model, although it also has some challenges to overcome. In terms of the generation, CRISPR-Cas9 gene editing via HDR is a relatively simple way to modify the genome as needed and create only the variant of interest while leaving the rest untouched. This results in a knock-in with a reasonable assurance that whatever phenotypes are displayed stem solely from the change in the SGCG gene. Specifically the muscle atrophy, elevated serum creatine kinase, progressive weakness (half the strength of controls at 4 months), and signifiacntly increased muscle fibrosis/necrosis are all hallmarks of the human LGMD presentation and are recapitulated here. However, the presence of a known muscular dystrophy-modifying background raises the question of whether some of these phenotypes would not be seen in a mouse with the C57BL/6J background. Unfortunately this is not answered in the paper, so it introduces some doubt as to the total validity of the results.

FamilyPreviousTesting: Authors stated that homozygosity mapping did not identify any significant CNVs. Loss of heterozygosity (greater than 1kb) were observed over all affected siblings in shromosome 2, 7, 10, and 14. The Chromosome 2q had a 11Mb region with 141 genes, of which none were associateed with ID, ptosis, and polydactyly together.

      FamilyMethod1:

      FamilyMethod2:

FamilyGenotypingMethod: Homozygosity mapping, using SNP array analysis ( Illumina Human-CytoSNO-12v2.1. DNA) on the entire family (including parents), and exome sequencing in all the siblings. All mutations were confirmed by Sanger sequencing in the proband, parents, siblings (affected and unaffected) and a control was used. WES identified mutations in the UGT1A7 gene (c.654A>C) within the shared chromosome 2 region, as well as ARMC( (c.879G>A). They also found mutations in the gene ZFPM1 (c.1331_1339delinsCCC), but this was not within any shared region of homozygosity between the siblings. Neither UGT1A7 or ZFPM1 is associated with a disease/ phenotype per OMIM.

VariantEvidence: The ARMC9 c.879G>A synonymous mutation is the last base in exon 9 (of 21 total exons) and is predicted to alter splicing. ARMC9 was transcribed into the pCAS2 minigene system and expressed in COS7 cells. RT-PCR of the transfected cells showed skipping of exon 9 in the mutant (Mt) cells compared to ARMC9 wildtype (Wt) cells that retained exon 9 (Figure 2). Sanger sequencing of the ARMC9-pCAS_Mt vector showed a 99 base pair deletion of exon 9 (Supplementary Figure 1), which is a loss of 33 amino acids encoded by exon 9. Exon 9 of ARMC9 encodes part of the ARM Domain 1 which is highly conserved among several speciies. Structural modeling indicates that the loss of the 33 amino acids (aa261-293) would critically disrupt the structure of ARMC9 ( Figure 3).

FamilyGenotypingMethod: A refined linkage analysis was performed with the deaths of some family members and additions of others, allowing for the narrowing of the genomic region. Afterwards, RNA was extracted from a frozen sample of the proband and cDNA was sequenced. PCR amplification and direct sequencing was used to confirm the mutation in the proband. SSCP was used to screen the family members and other NM probands for the mutation.

    1. Case 7In 1980, a 15-yr-old girl with a familial history of VHL presented with loss of visual acuity leading to the diagnosis of bilateral retinal hemangioblastomas followed by laser and cryotherapy. However, outcome was unilateral blindness. Furthermore, the patient was then operated three times for CNS hemangioblastomas between ages 18 and 24. At age 26, she presented with a palpable lateral neck mass. CT of the head and neck revealed a 3-cm carotid body tumor. The tumor was removed, and histology showed a paraganglioma. Molecular genetics identified the VHL germline mutation p.Thr105Pro.
