    1. PatientID: 9

      KindredID: 2

      Case: F, 29Y0M, African/African American

      DiseaseAssertion: VHL

FamilyInfo: She was 23 weeks pregnant at the time of a pheochromocytoma resection. Her 6-year-old daughter screened positive for the mutation; her 8-year-old son was negative. Both mother (although no testing conducted) and daughter were said to be investigated further and monitored long-term.

CasePresentingHPOs: HP:0006748; HP:0009713 (adrenal pheochromocytoma; spinal hemangioblastoma)

CaseHPOFreeText: At the time, the patient was 23 weeks pregnant and had bilateral adrenal phaeochromocytomas removed. There were no clinical features of VHL syndrome; however, MRI showed a haemangioblastoma in the cervical spine.

FamilyInfo: Families were selected based on their clinical data and family history. Patient’s father (BI1) had pheochromocytoma, but was not genetically tested. The patient’s son (BIII3) developed pheochromocytoma at the age of one, but progressed to bilateral pheochromocytoma 3 years later. Genetic testing revealed the same mutation in the patient and his son. In fact, a sample of amniotic fluid was taken from the patient’s wife (BII2) during the 16th week of pregnancy for prenatal testing, which was later confirmed to be the same point mutation as BII2. See pedigree in figure 1.

The following report is a 2011 updated pedigree on the previously reported Tisherman kindred. At the time of this study, 43/49 patients have undergone mutational analysis revealing the VHL familial mutation (Y112H) and 35/49 patients have clinical VHL manifestations. Phenotypic cumulative lifetime risk was 71% for Pheo/PGL, 15% for hemangioblastoma, 33% for retinal angioma, 3% for renal cell carcinoma, and 3% for pancreatic cysts. 6 members of the family (all deceased; 4 males, 2 females) had suspected VHL and 2 (males) of these were suspected to have had Pheo/PGL.

    1. A 32-year old man was invited to our centre for a consultation at the Polish VHL Registry as a member of a family with suspected von Hippel-Lindau disease (VHL). He had a history of left facial nerve palsy at the age of 19 years due to tumour of the temporal bone. The patient was operated on a few months later and a histopathological diagnosis of choroid plexus papilloma was made. Two years later he was reoperated on due to recurrence and treated with adjunctive radiotherapy. Three years later the patient developed ataxia and was treated surgically because of a cerebellar tumour that was histopathologically diagnosed as haemangioblastoma. Due to recurrence of this tumour the patient was reoperated on two years later. He also had a history of retinal capillary angioma treated by cryo- and laser therapy.

      PatientID: None

      KindredID: 1

      Case: M, 32Y0M, Ethnicity Unknown

      DiseaseAssertion: Assumed VHL

FamilyInfo: Many of the patient’s family members developed lesions in sites suggesting VHL including multiple central nervous system tumours (probable hemangioblastomas), renal tumours (probable renal cell carcinomas), pancreatic tumours (probable cysts and neuroendocrine tumours), temporal bone tumours (probable ELST), and some of the family members have lost their sight because of retinal detachment (probably in the case of retinal haemangiomas). However, the diagnosis of VHL has not been established in the family. Two of his children were also found to have the NM_000551.3:c.194C>T(S65L) mutation after analysis of blood samples.

CasePresentingHPOs: HP:0030393; HP:0010797; HP:0009711 (Endolymphatic sac tumour, hemangioblastoma, retinal capillary hemangioma)

CaseHPOFreeText: The patient had a history of left facial nerve palsy at the age of 19 years due to tumour of the temporal bone with a recurrence two years later (at 21Y). The patient was originally histopathologically diagnosed with choroid plexus papilloma, but was re-diagnosed with an endolymphatic sac tumour on reexamination of the sample. Three years later (24Y) the patient developed ataxia because of a cerebellar tumour that was histopathologically diagnosed as hemangioblastoma. He had a recurrence two years later (26Y). He also had a history of retinal capillary angioma.

FamilyInfo: Family 2 is comprised of 131 individuals over six generations including 65 members (50% of those at risk) with VHL type 2A (72% clinically affected, 12% obligate carriers, and 15% positive genetic analysis alone) and nearly equal gender distribution for affected status (54% male). An additional six members (II-3, II-4, II -5, III-1, III-2, and III-3) are suspected to have VHL without confirmation. Of note, the nuclear family (branch IV-13) contains 13 siblings, of whom 12 have been diagnosed with pheo. The presenting tumor types in Family 2 were pheo (70%), HB (17%), and RA (13%). No Family 2 member had RCC as the initial manifestation. Distribution of HB and RA are not described except for patient IV-8.

FamilyInfo: The patient's father (II:3) had a brain tumour at age 53 and died at age 78. The patient's paternal aunt (II:1) died of stomach cancer at age 80. The patient's paternal grandfather died at age 50 due to a brain tumour. The patient had seven siblings, five of whom underwent genetic testing. Two were found to carry the mutation but both were asymptomatic (patient III:2 and III:3). The patient's sister (III:2) had three children all of whom carried the P25L mutation and were asymptomatic (IV:4 and IV:6) except patient IV:2 who presented with a retinal hemangioma and renal cyst. Her niece, (IV:2) had three children and one was found to carry the mutation. The patient's brother (III:3) had six children one of whom died due to clear cell renal carcinoma at age 48 but did not carry the P25L mutation (IV:7). The patient had three unaffected children who did not carry the mutation. Of the 13 family members tested, V:2, IV:4, IV:6, III:3, and III:11 were all found to carry the P25L mutation in the VHL gene. Refer to Figure 1 for pedigree.

    1. What struck me about this data is that mere opposition to AI ranked down at the bottom of the list. But when data center apologists get the microphone, that’s the issue they seize on, while ignoring entirely the top concerns that people actually have. That makes sense, because there’s no getting around the resource consumption or impact on local communities, whereas calling the opposition Luddites is easy

Schleswig-Holstein is still using Windows, taking it step by step. MS Office to LibreOffice, Thunderbird w Open-Xchange for mail, calendar, contacts, Element/Matrix for chat, Nextcloud for collab.

    1. The strategy is to unbundle the parts of a package manager and standardize them individually. Registry APIs, dependency graphs, vulnerability feeds, update notifications. Each piece can be commodified without replacing entire systems. Eat the elephant one bite at a time.

yes, this is also how you tackle all the other silos. Deconstruct and recombine along different principles

    1. “There has been a pivotal shift in the last few months; we have seen a fundamental switch from just interest in sovereign solutions to actual buying behaviors,” said Maisto. “The demand is there. Organizations are investing money into sovereign migration projects, and we have not seen this before,” he said.

companies are voting with their procurement now

    2. In the end, most customers will opt for strategies that comprise a mix of sovereignty levels depending on their needs.

yes, during transition bc of pragmatics, same for my company, but while fully understanding the desired end-state, which is no big players USA or wherever based.

    1. The king of Kish even sometimes enforced order inSumer. For example, Enannatum’s son, Enmetena, wrote that theborder between Lagash and Umma had been determined by thegreat god Enlil himself and had been confirmed by the king of Kish:“Mesalim, king of Kish, at the command of (the god) Ishtaran, measured the field and set up a (boundary-) stone there.” Theauthority of the king of Kish was therefore acknowledged, at leasttemporarily, by both the king of Umma and the king of Lagash.

There is an interesting example of the mnemonic use of stone here in ancient Sumer. It serves as a boundary/border marker by its physical presence, but apart from any (other local) mnemonic uses, it also carries an inscription as a secondary form of long term written memory.

    2. The investment of time and manpower devoted to the constructionof this complex would have resembled the work on a medievalcathedral. As early as 3600 BCE work had begun on the so-calledLimestone Temple in the Eanna precinct. Quarrymen and masonsremoved limestone from a rocky outcrop around fifty kilometers (31mi) to the southwest. Other men transported the stone to Uruk. Stillothers formed hundreds of thousands of mud bricks and clay cones, and set them out to harden in the sun. Others brought timber fromfar to the north for the roofs. Someone supervised all the workmenwho set the bricks and stones and mosaic cones in place. The menwould have been fed and provided for during the construction. Thebuilders were all probably residents of Uruk, united in their desire tocreate a magnificent home for their beloved divine queen.

Possibility that even with proto-cuneiform (writing) evolving here that such temples were local memory palaces for the culture of the inhabitants who would have been primary orality-based?

    3. facts that no one couldconceivably commit to memory.

This statement belies the power of orality and the size of built communities without literacy. It's more a question of understanding how it was done and how communities either trusted (or didn't) those who memorized the materials.

Another factor is how long one needed to remember various facts, especially if for commerce and over what spaces?

Were there stratifications of society based on the power of memory here? Compare the anthropology and archaeology with the studies by Lynne Kelly.

    1. I am hopeful. Not optimistic. Fuck optimism! Optimism is the idea that things will get better no matter what we do. I know that what we do matters. Hope is the belief that if we can improve things, even in small ways, we can ascend the gradient toward the world we want, and attain higher vantage points from which new courses of action, invisible to us here at our lower elevation, will be revealed. Hope is a discipline. It requires that you not give in to despair. So I'm here to tell you: don't despair.

hope, Vgl my talk #sotn18 where I said the same thing. It's an action

    2. t's national security hawks who are worried about Trump bricking their ministries or their tractors, and who are also worried – with just cause – about Xi Jinping bricking all their solar inverters and batteries. Because, after all, the post-American internet is also a post-Chinese internet!

      Natsec as driver, als post-Chinese

    3. But crises precipitate change. Remember when another mad emperor – Vladimir Putin – invaded Ukraine, and Europe experienced a dire energy shortage? In three short years, the continent's solar uptake skyrocketed. The EU went from being 15 years behind in its energy transition, to ten years ahead of schedule.

True? Did we see acceleration in solar since early 2022, shortening the timeline with 10 yrs? What was the sched anyway?

    4. the project of building a post-American internet: a project to reduce tech debt, to unlock America's monopoly trillions and divide them among the world's entrepreneurs (for whom they represent untold profits), and the world's technology users (for whom they represent untold savings); all while building resiliency and sovereignty.

post-American internet as a project reducing tech-debt (meaning in this context?)

    5. But there's one post-American system that's easy to imagine. The project to rip out all the cloud connected, backdoored, untrustworthy black boxes that power our institutions, our medical implants, our vehicles and our tractors; and replace it with collectively maintained, open, free, trustworthy, auditable code. This project is the only one that benefits from economies of scale, rather than being paralyzed by exponential crises of scale. That's because any open, free tool adopted by any public institution – like the Eurostack services – can be audited, localized, pen-tested, debugged and improved by institutions in every other country.

digital transition is possible because it scales through spreading. You don't have to solve exponential scale first.

    6. Any serious attempt at digital sovereignty needs migration tools that work without the cooperation of the Big Tech companies. Otherwise, this is like building housing for East Germans and locating it in West Berlin. It doesn't matter how great the housing is, your intended audience is going to really struggle to move in unless you tear down the wall.

Building alternatives only useful if you have a guaranteed path of migration. Bigtech will not provide it.

    7. We need scrapers and headless browsers to accomplish the adversarial interoperability that will guarantee ongoing connectivity to institutions that are still hosted on US cloud-based services, because US companies are not going to facilitate the mass exodus of international customers from their platform.

this is a good point. Bigtech will not stand by as customers move out. So you will likely must have circumvention tooling, ... which is illegal.

    1. These cases illustrate that while a complete break from US hyperscalers may seem unrealistic, as Forrester predicts, targeted migrations for specific, high-risk applications are not only possible but are actively being pursued. They represent a grassroots movement, driven by legal obligations and a growing desire for autonomy.

      another odd sentence.

targeted migrations is how you break from hyperscalers. Not by expecting to create your own hyperscaler (which would also then be a potential monopolist and threat to digital autonomy if not sovereignty like US stuff)

It's not 'grassroots' if national government entities are switching. What would be top-down in this case?

    2. The scale of Europe's technological deficit makes migrations like Austria’s daunting for most organisations.

non sequitur. You just had multiple paragraphs saying it was easy for Austria. Most organisations in Europe are small, making migration easier.

Yet,at the same time, we have living elements that at one time in history, were independent living beings which were NOT part of a multi-cellular colony! - We have genes, that were once part of autonomous living entities, - Mitochondria within our cells at one time were autonomous entities, and cells, which were also once autonomously existent eukaryotes. All three exist in transmuted form that is now integrated into our body.

    1. though he claimed to be nothing more than a translator – therebyconforming to medieval literature’s aversion to originality, at least outwardly –he remains one of the most strikingly original writers of the Middle Ages.

And isn't this just what we would broadly expect of a culture moving from primary orality into literacy? The old wisdom passed down through memory would be prized over the invention of the new.

Until the written tradition can be fully relied upon, originality wouldn't be prized the way it is in current Western culture.

We should expect writers to say (even if lying) that their material is based on prior sources and not created by them.

Of course perhaps the primary oral bards may have exhibited this pattern themselves and used their own creativity to embellish prior "close truths". Just how far did originality go in the bardic tradition?

Compare this with using written statements to actively tell lies in the opening centuries of literacy.
